2yo DS complaining about perineum pain?

My DS is complaining that his bottom hurts and he showed me where which is really his perineum (skin between balls/anus).  He has pooped like 4 times today but I don't know if that is related.  Any thoughts as to what this might be????

 He is potty trained except naps/night so I don't think it is diaper rash/irritation.

    Could he need a shower? After pooping 4 times maybe he just feels a little irritated or raw down there.

    Was it hard stool? Could he have strained and experiences a anal fissure?

    ditto, Carson isn't potty trained yet but anytime he poops more than once (or sometimes just once) after wiping and getting him clean his little perineum is always red, i use balmex and it looks fine by the next diaper change.  all the enzymes in poop can make the skin irratated.
