Mono/Di Twins: How will/did you deliver?

I am expecting Mono/Di Twins (shared placenta), and my OB said that he would recommend a c-section to avoid risks to the babies. He said that if I were to deliver vaginally, there is a risk of the placenta breaking down after A is born, putting B in danger.

Anyone else expecting Mono/Di twins, or who has already had them? How did you deliver, or what has your doc said about delivery options? I'm certainly not willing to risk the babies' health if a c-section is best, but just curious if this is pretty standard, or more of an opinion/depends on the doctor thing.

  • My girls are not here yet, but mine are mono-di and share a placenta.  While I am aware that it is likely that I will have a c-section based on position, I was never told that it was dangerous to possibly deliver vaginally.  If both are head down my OB would let me go naturally.  Personally, I have a HUGE fear that I would deliver A vaginally and B would require a c-section.  My babies keep switching, but as of yesterday B is lower and would come first if we delivered now and she is butt down, so at this point, it would be a c-section.  My OB did say that they would consider a breach delivery for the second baby IF the second baby was smaller than the first.  I keep pushing her for a plan, and she just keeps telling me we need to wait and see what their position is.  I am curious to hear what other MoM's say about how their delivery's have gone with one placenta.  
  • I am going to schedule a c-section.  A peri in the group I go to said he would do it at 36 weeks, but my doctor might want me to go to 37 weeks.  I have to ask him when I see him next.

    After 2 rounds of IVF & 2 rounds of FET, we were blessed with identical twin girls!

  • It's interesting that you posted this today because I was JUST reading last night on the TTTS Foundation's website about Acute TTTS happening during labor.  It said that if vaginal delivery is attempted in monochorionic pregnancies, that the babies' heart rates should be closely monitored for signs of distress.

    I had already been considering a c-section based on my tearing, blood loss, and GBS-positive history with previous births... but this makes me lean even more toward a c-section.  Just scary.

    Addition:  Here's where I read that info.

  • I have mono/di twins. I'm being closely monitored by my perinatologist, but so long as the placenta remains healthy we plan for a vaginal birth. There would have to be something seriously wrong for me to even consider a c-section.

    My OB wanted to induce since Baby A was head down, but Baby B was transverse, and the docs on call were more comfortable with a C-section (as was I at that point).

  • My boys were mono/di. I was complete and pushing for about 45 minutes when both boys' heart rates dropped and they called for a c-section. I never heard that it could be more dangerous then any other twin pregnancy. You just need to make sure both baby's heart rates are being monitored at allll times!

    When we didn't know if they were Mono/Mono or MonoDi I asked my OB  & Peri about this and they both said this!  If the first is in postion, & the second is not transverse, they would each do a breech delivery for the 2nd. But there is are always risks of having a section due to heart beat issues etc.  If you placenta is breaking down, it would result in a low HB for Baby B & they wouldn't know which to go with until the position of the babies were known.

  • Mine are possible mono/di (never confirmed if there are one or two placentas) and the Dr. will deliver me vaginally if both are head down. He did say that because Baby A is smaller that if they think vaginal delivery is too traumatic for it, for whatever reason, they will do a C. As of now, we are just waiting to see what positions they get in. A has been head down and be transverse/breech so just gotta tell B to turn.

  • My boys were mono/di and I delivered both of them vaginally.  They were both head down so we went for it and had a good delivery.

    Thank you for sharing your story.  This gives me some hope, at least of they are both head down! 

  • My boys were mono/di both babies were head down  with the bigger twin in position to come out first- both were delivered vaginally with no complications :)

  • Most doctors at my practice pushed for a c-section - b/c of the placenta breaking down, stress to baby B, blah, blah, blah.  But the two older doctors there told me there was no reason not to try vaginal if both babies were head down.  They were and I had a very uncomplicated vaginal delivery for both babies.
  • My girls were Mono/Di and both head down. I pushed for 1hr 20 minutes and Baby A was delivered and only 7 minutes later I delivered Baby B with no complications.
  • I plan to deliver vaginally if all goes well.  My OB hasn't expressed a concern about my twins sharing a placenta, as long as they are both in good positions.

  • I delivered by C, and thank God I did. After Baby A's cord was cut, my placenta started detaching and Baby B went into distress. His initial APGAR was 2, and he had to be revived in the OR. He would not have survived a vaginal delivery, he barely survived the C.
