I didn't use them until Savannah smacked her head on the side of the crib multiple times and woke up screaming each time. I decided then the bumpers were going on the crib. Since then i've had no issue with her bumping her head and she stays in the middle of the crib so we've had no issue with her face being in the bumpers either.
I started using them when she hit 3 months. Mostly because I noticed her crib was getting very drafty since we had the cool mist humidifier going with the ceiling fan. She would just get a constant breeze and her mouth was very dry when she'd wake up. I put the bumper on and she's been fine. She doesn't roll over yet, so I'm not worried about her getting face-planted.
We used bumpers until last night. I woke up to Gianna grunting and saw her face was against the bumpers. She could have turned her head to the other side but she didn't. That scared me enough to take them off this morning. She is also spinning around in her crib. She's still swaddled so I think it's time that they go. For now at least.

We have breathable bumpers, but took them off pretty early on since they're such a royal PITA. We might put them back on if DS starts flinging his arms/legs through the rails, but so far the most he's ever done is roll his head into the slats, which would do just as much damage (not much) with or without the breathable bumpers. I actually put his seahorse about where he tends to roll his head, so he doesn't bonk it (the seahorse is contoured/plastic enough so his face never gets in the material.)

I used the regular bumper until he was 3 months old and showing signs of more movement. Now I use a breathable bumper.
(DS was in a sleep positioner so he could never "move" until I took that away).
