GD: diet controlled vs. insulin question

If you are on insulin, do you deliver earlier than if you are just diet controlled?  Just curious if the insulin dependance changes anything...

  • When I was first diagnosed with GD back in February (i was only 6wks) my doctor told me it was his standard protocol to induce at 39 weeks for all GD patients.

I had not brought it up since then, until two weeks ago, and he re-phrased it when I asked and said "yes, since you are insulin-dependent we will induce at 39 weeks".

So I am not sure if he would still have plans to induce a week early if I had been diet-controlled.

I also don't understand what insulin-dependence means for the placenta.... which is the only reason I can think that would give him cause to induce early.

I thought I had read somewhere that with insulin, the placenta breaks down faster.  So someone that was normal, it would break down at 42 weeks, but insulin causes that early which is why women with GD are induced early.  I could be wrong, but that's why I posted.

  • I don't know but am curious.
  • I was told that since I am insulin dependant, not diet controlled, I would be induced NO LATER THAN 38.5 weeks. I was told it was because of the fast deterioration of the placenta, though I dont really understand how the insulin does that.

  • I was on insulin 3 times a day and now I'm on it only once but my doctor will not induce unless she sees something wrong...she keeps telling me that I am very healthy and having a very healthy pregnancy and she will let me go past 40+ weeks.

So to answer your question IMO - I think it depend on an individual basis and your doctors judgement/ protocall. I've learned every office is different.

  • I was reassured to find out that my ob doesn't have a firm deadline that she won't let me go past. She normally prefers to avoid induction when possible (she said it's because they increase the likelihood of a c-section). That being said, they're keeping a close eye on me. I have non stress tests twice a week now because I've been put on insulin. Apparently the non stress test is monitoring the placental health (through monitoring the baby's health). So, if my placenta does start to break down early, they'll know quickly and can get me into labor. But, if everything seems healthy with baby, including his size, there shouldn't be a problem with letting me go to a normal number of weeks.
