Morning girls! I know the Azoospermia check in is on Thursdays but I had a question. My DH had a SA last May and we found out that there are 0 sperm so we were referred to a fertility specialist who ran tests. We found out that it's nothing hereditary but his testosterone levels are low so we were then referred to a urologist. More bloodwork and an ultrasound later, he has small dialted veins around his testicles which the urologist doesn't think is the cause for the 0 sperm. The Dr. says that he will most likely put him on a gel for the low testosterone, which sounds fine but after googling I keep finding information saying that the gel actually lowered sperm counts. Have any of you had any experience with this? Also, can you go from 0 sperm to normal after taking this gel? Thanks in advance, have a great Monday!!

I think the dilated veins are probably varicocele? They are probably not the main factor in the azoo, but if they are severe I think the uro would try to fix them prior to a biopsy/tese.
If that's Androgel - I think that while it will increase the testosterone, it could prevent sperm production (even without the azoo factor). I'd see if there were other alternatives until you guys figure out what your next step will be. (ex - some uro's won't provide testosterone supplements until after a failed mTESE where the are confident there is no chance of finding sperm). I don't know a whole lot about this, as our uro hasn't discussed treating the low test. yet.

MY DH had low-ish testosterone and was sent to a urologist after one of his annual check ups. When the urologist found out we were thinking about TTC (at the time) he refused to prescribe any testosterone. We recently met with an RE for the first time and discussed DH's low testosterone issue from before. The RE asked whether he had been given testosterone supplements and when we told him no he said "thank god". The RE explained that testoterone supplements affects sperm production and is very bad for male IF. I know that seems a little counter intuitive, and of course your DH's situation may be different than mine, but wanted to share.
My DH had low testosterone as well as a vericocele and hydrocele. He had the surgery to repair and has had a major increase in his testosterone and some improvement in overall numbers. He is currently on Clomid to help the numbers continue to increase. Low testosterone has really bad health effects and needs to be addressed. Maybe time for a second opion or a Uro that specializes in fertility.
