I am confused- re: low blood sugar

I was on a super high dose of glyburide (10mg/day), and suddenly, overnight, my numbers fell. I went from *barely* being controlled during the day to getting 2-3 lows. They dropped my dose to 5mg/day- still had lows. Now I'm on 2.5/day, and probably will be off the glyburide totally on Monday. My 2 hour numbers are under 100 now.

My confusion- my OB was VERY concerned about me having lows (below 50 even). She said they might need to admit me to the hospital to make sure I wasn't going to crash, and to see if they could get me more stabalized. She called my perinatologist though and I guess the peri isn't concerned? The peri said 60 at 2 hours pp is "normal", but feels weird to me since I've had such highs. 

I didn't question it much at my appointment, b/c who really wants to be admitted to the hospital? But when I got home and googled (bad, I know) it said Hypoglycemia is considered anything under 70, and that numbers under 50 can impair brain function! I had a low of 54 about 1 hour after eating WHILE at the OB's office... and they aren't concerned? I'm just confused, google says low BS is just as bad for the baby...

    I don't have any advice but to me it sounds wrong! My midwife and my GD counselor told me that I will probably need more insulin as the pregnancy progress because your body is working harder towards the end. My numbers have also declined. Not as drastically as you. I told my OB on Friday and yes he was concerned because he said the placenta might not be working like it should and any changes to go immediately to the L & D. He lowered my insulin dosages and I see him again on Monday.

    I really don't think a 60 2 hours after meals is normal! I get the shakes and feel horrible sometimes when I am in the 70's or 80's if I don't eat really quick! Anytime I am in the 60's or 50's I am miserable!!

    I have also read that crazy fluctuations are bad for the baby.

    I say go with your instincts and talk to your OB more. She seems to have yours and the baby's best interest at heart!

  • The good news is my OB is going to induce me soon! She was okay with 38 weeks, but the perinatologist wants 39 weeks... it's just weird that my OB was so concerned, concerned enough to mention admitting me to the hospital, and the peri said it was normal!?!?

    I'm going to ask some more at my next appointment, it's just SO weird that I would go from needing the max dose to needing nothing in less than a week. It makes me very nervous!

    I really feel okay until my numbers hit 60's... if I catch it in the 60's I'm okay, but if it drops below that then I feel AWFUL. 

    All of my 2 hour numbers today were under 100.. I forgot to check my keytones, my OB said it's possible my body is using up some of it's stored insulin and by limiting my carbs I'm actually starving myself! Eeek! 

     

  • Are you having NST's and BPP's?

     My OB told me (type 1, on insulin) that in the end, a drop in insulin needs may indicate that the placenta is beginning to deteriorate, which is common in diabetic Mom's. Placenta beginning to deteriorate = fewer hormones released to counteract insulin = lows. My Peri told me that a 38w placenta in a diabetic mom is like a 40w placenta in a non-diabetic mom.

  • I had 2 NSTs last weekend, and I have an ultrasound this week... based on the ultrasound they will decide when they are going to induce or schedule a c-section. At my last ultrasound, Emmy's abdomen was measuring considerably larger than the rest of her... I guess if the ratio is too off, they will not try to induce b/c of shoulder dystocia.

    I want her OUT so badly, I am on bedrest for pre-e as well and I feel like every day she is in there just ups the chances of something going wrong :( I'm a kick count fiend though, so hopefully I would pick up on it... 

  • Very interesting info about diabetic moms and the placenta. That's what I thought but wasn't sure.

    I am so glad your OB is taking good care of you!  I too will be induced soon. We are talking to the doc about that on Mon.

    I didn't realize that we have the same due date, 8-24-09. We will have to see who gets to meet their LO first!
