I like a lot of "old lady" names (including Irene, which DH hates with a passion), but for some reason I really don't like Hazel. It just seems very old fashioned to me.

I like it - in an old-fashioned but classic kind of way.
As far as middle names, I'd go for something with 2 or 3 syllables (like Alexandra) since Hazel is on the short side.
It's ok. I'm just indifferent, I suppose. I can see this name becoming popular along with Mabel.?
I much prefer Jane and Elizabeth in your name poll. ?
I don't know about Hazel being trendy...
