  In the first season, he was given the first opportunity to scrub in to a surgery. He choked.

  They laughed and called him 007---with a license to kill.

  Every once in a while, they still refer to him as that. I think earlier in the episode Alex called him that.

  The first season, George was the first to help in a surgery.  I think it was an appendectomy...  Anyways, a pretty simple procedure nonetheless.  Well, he almost killed the guy.  Hence the name 007 (license to kill).
