I EBF and was just started on the z-pack (azithromycin) for respiratory infection/asthma. I asked the dr about breastfeeding while taking it and after getting out his giant med reference book there wasnt a cut and dry answer he could give me. He suggested at least pumping and dumping for the first 24-36 hrs since the first dose of the med is a double dose. He said after that he thought the breastmilk should be ok, but just to watch for GI upset/colicy signs (which DH is freaking out about) I have a freezer stash that could possibly get me through the 5 days of the med. But I didnt want to pump and dump any longer or even not long enough than i had too. Thanks.
And DS is 8 months and 18lbs if that is factored in.

I was bored and can't sleep so I looked up some studies and it does accumulate in breastmilk (2.8 micrograms/mL following the initial dose of 1 gram and 3 doses of 500 mg in one of the studies) but the amount absorbed by the infant isn't clinically significant.
I wouldn't worry...
My PCP is incredibly knowledgeable and understanding when I comes to my concerns with BFing and meds. I recently had a sinus infection and she was going to treat it with a Z-pack, so it MUST be safe. I wouldn't worry about dumping, either.
Instead, she put me on another antibiotic that would be more helpful towards eliminating the infection. With BFing, I think our options for meds are limited anyway, so I hope this one works for you!

Always talk to a LC about this! Drs and even pharmacists aren't that kowledgeable about BFing and drugs.
When I had a sinus infection I talked with the LC about what I could take. Z pack was one of the ones she said was really safe and recommended I use. No need to pump and dump at all.
