? EBF and 'z-pack' antibiotic

I EBF and was just started on the z-pack (azithromycin) for respiratory infection/asthma. I asked the dr about breastfeeding while taking it and after getting out his giant med reference book there wasnt a cut and dry answer he could give me. He suggested at least pumping and dumping for the first 24-36 hrs since the first dose of the med is a double dose. He said after that he thought the breastmilk should be ok, but just to watch for GI upset/colicy signs (which DH is freaking out about) I have a freezer stash that could possibly get me through the 5 days of the med. But I didnt want to pump and dump any longer or even not long enough than i had too. Thanks.

And DS is 8 months and 18lbs if that is factored in.

  • I was bored and can't sleep so I looked up some studies and it does accumulate in breastmilk (2.8 micrograms/mL following the initial dose of 1 gram and 3 doses of 500 mg in one of the studies) but the amount absorbed by the infant isn't clinically significant.

  • My PCP is incredibly knowledgeable and understanding when I comes to my concerns with BFing and meds.  I recently had a sinus infection and she was going to treat it with a Z-pack, so it MUST be safe.  I wouldn't worry about dumping, either.

    Instead, she put me on another antibiotic that would be more helpful towards eliminating the infection.  With BFing, I think our options for meds are limited anyway, so I hope this one works for you!

    No way would I dump, but that's just me!  Feel better!

  • I would pump and dump after that initial big dose for a day or so - then resume bf'ing after that (or pump and dump for a few days - and then resume when you are almost done with the z-pak.)  I only say this because not only do anti-b's kill all the bad bacteria, they also kill all your good bacteria, and can set you and baby up to get thrush.  I had this happen twice when on anti-b's - one time I still fed the breastmilk - the next time I pumped and dumped but LO STILL got thrush (and so did I) the second time because he got just a little milk with anti-b's before I started dumping it.  You can take probiotics (and should!!) to help to ward off the thrush for both of you.  GL, I hope you're feeling better soon.

  • i'm allergic/sensitive to a lot of antibiotics- zpack is one of the few that doesn't affect me.  i had to go on it for a tooth infection when DS was 3 months old.  i checked with the lactation consultant, my doctor, my ob-gyn, my primary care, AND the pharmacists at 3 stores.  all of them approved it for me and i never pumped and dumped.  ds had no problems with it either.
  • Always talk to a LC about this! Drs and even pharmacists aren't that kowledgeable about BFing and drugs.

    When I had a sinus infection I talked with the LC about what I could take.  Z pack was one of the ones she said was really safe and recommended I use.  No need to pump and dump at all.

  • I just finished a z-pack for a double ear infection. ?Yeah - I was the one with the double ear infection. I didn't pump and dump and DD is fine. ?She was also on amoxicillin for an ear infection at the same time and the drs and pharmacists weren't concerned about the extra antibiotic. ?In fact, they think it helped her get over it faster. ?GL and feel better soon!
