If mine wants to do one I'll refuse unless it's to check amniotic fluid or something...weight u/s is generally inaccurate and a great way to get yourself on the operating table for no good reason.
i'm very afraid of having a very large baby, not really because i think it will hurt more but because i'm afraid she will get stuck. i think that is my biggest fear as of late.
Unless there's a reason, I don't think I'll get one either. My ob is pretty low maintenance on u/s. If all goes as planned, I'll only have on this entire pregnancy.
I think they can usually tell by feeling. My OB predicted my friend's baby's weight within 2oz just by feeling.
I have growth u/s, but just because I have high BP, which can cause IUGR and I have "poor maternal weight gain" from being so sick. They're not really trying to predict weight, but just to make sure the baby is growing because, if she stops, I'll have to deliver early.
OB doesn't do one, either, unless there's a medical necessity.
My sister pushed out a 10lb2oz baby, so they figured I'd be able to push out a larger sized baby if needed. I pushed for 2 1/2 hours and she wouldn't fit through my pelvis and DD was only slighly over 9lbs, so you never know how big a baby you're able to push out. It all depends on the shape of your pelvis. It's not really something you can prepare for! ;-)

Leopold's Maneuver (where they feel how big the baby is through your belly) is a more accurate estimate of weight than an ultrasound (with an experienced practitioner--which I hope your dr is). Estimated fetal weight by ultrasound can be +/- 2 lbs.
Rarely is there a true case of a baby being too big to fit, and there are many signs of that which often occur before you even get to pushing. Your body is made to do this, pregnancy in itself is natural, not a medical condition.

the doc will be measuring your fundal height at each visit and i imagine that if you were measuring way big consistently then they might give you an u/s to check growth - however as many pp have noted, the estimates on growth u/s are notorious for being way off.

There's your answer. There's really not much you can do about it, except to eat a healthy, well-balanced diet. If you live off of cake and ice cream, you may end up with a bigger baby (calories make mommas big, sugar makes babies big).

Have you considered chiropractic care? It's really helpful in helping those pelvic bones move out to make room for baby. I was really worried about a breech baby because almost every woman in my family had a breech baby and/or c-section. I started with chiro care early and often and I had no problems pushing my DS out!
My OB doesn't do third tri ultrasounds either. They can estimate your baby's size based on your fundal height (measurement of your uterus in centimeters). The ultrasounds can be off 1-2 lbs either up or down so they don't really tell you that much either.
If you're really concerned about having a big baby, pay attention to what types of foods you're eating. I was almost 10 lbs and had shoulder dystosia, so I'd like to avoid that at all costs with my baby. My midwife said to make sure to avoid the bad carbs (cookies, cakes, breads, etc.) and make sure I'm eating lots of veggies, lean protein and not going overboard on the fruit. I know it's easier said than done, though.
