I heard that if you hold the valve and membrane upside down and there is a big gap where the membrane usually lays flat, then it's time to replace them. I actually did this test this morning and noticed a big gap but didn't have any issue with output, so I just ignored it for now. It's been about 2 weeks since I replaced mine, so I figured I could wait and just try to do it once a month.

I only changed mine if I noticed the nubby part tearing away from the disk. I EP'd so this happened pretty regularly, but no more than every couple of months.

I replace mine every 2 months as a precaution. I haven't noticed a substantial difference in output, but lots of women do. If you have never changed yours yet, I would.
I exclusively pump and replace mine every two months or so. If you notice that your supply is going down a bit I say that replacing it is worth a try. I also recommend replacing the tubing as well.

I just go by feel. If my pump is set at the same suction, but it feels less suctiony, I do it. I've changed them twice in the three months that I've owned the pump, but my usage just greatly increased when I went back to work.
