survey respondents needed for new after school program

Are you a parent? Do you know a parent? We're looking for parents of kids between 5 and 17 years old to give us their opinions on their kids' after-school activities. For each completed survey linked below we will contribute $5 to our own scholarship fund that helps those who cannot afford to attend our programs on their own.
