23 growth discordance?

I had my monthly growth ultrasound yesterday and baby A is measuring 36oz and baby B is measuring 42oz.nbsp; The past 3 ultrasounds since 18 weeks they were always within a couple of ounces.nbsp; I was able to access my u/s report online and it said that perhaps the discordance is due to greater than average growth of baby B rather than stunted growth of A.nbsp; I don't see my OB again until next Monday and my question for you all is, would you ask for a f/u ultrasound sooner than 4 weeks assuming the OB doesn't mention it? I feel like the next four weeks are crucial for weight gain and I don't want to miss anything.nbsp; nbsp;I am having di/di twins and the only problems thus far have been PTL, elevated B/P at times and low platelets I have a hematology appt. tomorrow. nbsp; Things have calmed down the past couple of weeks and I was able to return to work and feel better than I did. I am not being followed by a MFM, the closest one is two hours away and the OB never mentioned it.nbsp; Any suggestions or experience is appreciated!nbsp; nbsp;

Edited to update: I just received a call from OB office they are referring me to a MFM at the University of Washington about 2 hrs away. So now my question is answered. Hoping to get the appt set up soon. Thanks again!

  • I dont know the exact measurements anymore but my A has always been bigger and at the 20 week ultra sound there was about 2 weeks difference. Now B is bigger as of 36 u/s but by only a few days. No high bp no complications, didi boys.

  • Discordance is less of a worry with di di twins.  Our u/s tech basically explained that di di is essentially 2 singletons so its to be expected that they will grow differently, just as someone who had singletons wouldn't compare the gestational weight estimates of their older child with the one they are pregnant with.  I didn't explain that as clearly as I would have liked, but I hope it helps.

  • My twins are di/di and around 28 weeks their growth measurements were drastically different. They measured over a pound and six ounces different. My MFM was not concerned. At birth my baby A was exactly 2 pounds heavier than my baby B.

    1st u/s on 10/10/12: It's Twins! We found out on 12/16/12 that we were having two BOYS!
    C & D were born on 5/16/13 at 10:46 am and 10:47 am

    This. Di/di twins are likely fraternal [although not always] so they are no more similar genetically than siblings born years apart. Growth differences are usually totally normal. But talk to your OB or MFM if it will ease your worries!

  • Mine are a week apart after my last appointment and they scheduled the u/s for three weeks later rather the four for follow up based on that.

  • Even thought they are di/di I would still ask for another growth scan in 2-3 weeks.  4 weeks is too long to wait IMHO.
  • I wouldn't worry about it. We had the same issue - our doc calculated 30%, mfm calculated 24% then redid some measurements and got 20%. Then, the next time who would have thought - the smaller one had magically met and surpassed the weight of the larger one! It is all such a guess. Additionally, the mfm told us that since B was transverse during the earlier calculations, that can produce skinnier head measurements that can skew the results. He was head down the next time when they said he caught up.

  • Wow, at my last growth scan (32 weeks) mine were 3lbs7oz and 3lbs8oz.  I would definitely say that baby B is just growing more than average. 

    The scan before that, they had a greater discordance but leveled out again by the last one.  

    I would definitely talk to your MFM about it if s/he doesn't bring it up.  Because I am the way I am, I would probably ask about a f/u u/s also.  But that is just me.

    +#2 Tx cycle 4 - 5 mg Letrozole + 75iu Follsitim & Ovidrel w/ IUI on 11/13/12 - EDD 7/23/13 
    7 week u/s revealed THREE babies, all with heartbeats.  153bpm, 148bpm, and 136bpm
    9 week u/s revealed loss of Baby B.  A and C are growing on track.  A measuring 9w1d with 172bpm and C measuring 9w0d with 179 bpm.  
    Elective sex determination u/s on 2/8 revealed... boy AND girl!

  • I'm having di/di boys. I can't see how far along you are, but at my 31w growth scan they estimated A at 3lbs11oz and B at 4lbs1oz. As they've grown, their disparity has gotten larger which I think is to be expected as the measurements become less accurate. What my dr has been focusing on is that they both are continuing to grow along a curve, not nec that they are hitting certain weight milestones. Remember also at this point that weights can be off a pretty sig amount.

    I'd bring it up, but I don't know how concerned I'd be since they are di/di. But I know how hard it is not to worry.

  • Thanks for all of your replies! I am 30 weeks 2days for those that are on mobile. I know it is less of a worry with Di/Di but still concerned because not only was weight off but A is measuring 2 weeks behind B. they have always measured the same. I still think B is just big for gestational age but I will ask the OB on Monday.
