Separation anxiety

At first it was me, not its him... We were doing really well leaving him with sitters, always family, but then he got sick. He got a double ear infection so I didn't leave his side and them he had surgery so he was spoiled and then back ones for one ear that wasn't fully healed. Now, we can't leave him. He will completely freak out even if he's at home. Please help! What is a good transition? I would like to be able to leave him occasionally for a few hours to do dinner or a movie with the hubs.
