My DD keeps asking to play with her friends outside of school. I very rarely see other parents at school bc I drop her off later, and MH picks her up. School does not give out a parent contact list. I was thinking of leaving little notes in the kids' cubbies for the moms with my phone number on it.. Is that cheesy or weird? Or would you be excited to be able to plan a play date with one of your kids' friends?

I agree, I'd be excited too!
However, try not to be discouraged if not everyone feels the same way. Some people have limited time for play dates, or are just disinterested.
I love planning playdates for DD and her friends. Granted, so far I've only planned with parents I have met, but if my DD asked me for a playdate with a kid whose mom I didn't know, I'd do exactly what you're planning to do.

I was excited when ds' friend at school wanted to play afterward and I got a note in his cubby. I went to 2 play dates and then later ds was invited to his birthday party.
It was a good experience overall, but the mommy friend chemistry between me and his mother sort of fizzled after the first play date. Second play date and birthday party were a bit uncomfortable because we didn't have much to talk about. So, now I'm a bit more shy to try to initiate play dates with ds' friends in this class. I'm hoping kindergarten in the fall will bring better success.
Leaving a note in the cubby is a great idea. Is your LO shy or loves to talk? My DD will flat out ask for a play date and suggest she call her mommy. She will also offer the news that there is a note in the cubby about a playdate.
Also, if they are a WM and wknds are tough, you may suggest a get together after school at a CFA or MCD's playground. You can knock at dinner at the same time.
