Secondary IF

New Lurker with Question

Hello. I have one DD who is 4. We have been trying to conceive #2 for 18months and were recently DX with unexplained IF. We have 3 failed responses to clomid and my doctor wants us to do injectables.

He recommends Follistim 75iu cycle days 3-10 and trigger with Ovidrel. He said he will do BW and US on cycle day 6 and tell me when to trigger, based on follies growing at 1-2mm/day and my E2 levels. Then TI no IUI.

Everything I read shows more monitoring involved. I'm just scared to move on to injects without proper monitoring. No Kate plus 8 here! Or OHSS!

    It is very hard to get a lot of kids now a days, since most clinics only transfer 2 embryos.

    Also about OHSS, is you are motinored well and drinking a lot, you can try to avoid OHSS.

  • I don't mean to be a jerk but I think mlamping is somewhat incorrect.

    With IVF it is rare to get high order multiples as many clinics will only transfer one or two. Mind you I know a quad mom who only transferred two. Identical triplets and a fraternal quad.

    With superovulation/iui you do run a higher risk of high order multiples especially if not monitored. I think your 1 ultrasound at CD 6 is a bit careless... I have had cycles with only 1 ultrasound most of mine really but later in cycle right before maturity/trigger and we had a pretty good idea of how many will be mature. To me CD6 seems way to early to accurately predict this. I would ask for one later or a second later. Just my two cents.. Or five now due to loss of penny ;

  • Welcome!  I am a grad of this board but lurk.  I hope it is ok for me to respond.

     I think mlamping misread your post.  If you are doing TI CD6 is way too early for monitoring.  When I did my 3 IUI I was monitored right before I triggered (like days 11 or 12) so they could see if I had follies, size, how many.  I really don't like your Dr approach of "guessing" they grow 1-2mm per day.   

    When you say "failed responses" did your not produce eggs of the right size? Did you not ovulate? or did you mean you just didnt get PG?  For me i didn't respond well to meds. Sometimes I either produced nothing or way too big.   

    Are you working with an RE or an OB?  Why not IUI? we did IUI with the idea of getting all the swimmers exactly where they need to be.  We had no male issues and were basically unexplained except for my age, 39.

  • I also think you should be monitored more.  When I used Follistim and Ovidrel, I went in M, W, F to do bloodwork and ultrasounds.  This way they could adjust the dosage if needed and to watch the follies-to see the size and how many.
