Hi ladies,
I'm scheduled for IUI #3 on Friday. It's our first with injectibles. I've been on Menopur for 15 days and I have 3 follies. Today two were 17 mm and one was 16. I'm doing one more night of menopur, then triggering tomorrow. We also have MFI but are using two frozen (16 mil. and 1 mil.) and one fresh sample. Yesterday my doctor had a discussion with me about our chances for multiples. He said we have a 20% chance of twins and a 1% chance of triplets. He wanted to be sure that selective reduction is something we would consider if I did get pregnant with 3+. He was not willing to continue with the cycle if I wasn't open to that. Has anyone gotten pregnant with multiples through IUI when there's MFI and three follies? I'm just so used to it not working and the fact that he took the time to discuss the importance of this issue is making me more positive about this cycle. Sorry for the long-winded post!

3 mature follicles would definitely make me nervous.
That said in all my inj cycles I only ever had 2 mature follies. When I triggered with DS2 and this baby, only one follie took each time. We did not have MFI.
However, I don't think I would necessarily reduce from 3 just because the RE wanted me too. I would definitely consult with an OB and MFM before making that decision.
When TTC DS we had three Femara/trigger/IUI's with 3 follies each time which resulted in two BFN's and then a singleton pregnancy. This time it took two rounds of Femara/trigger/TI to conceive this baby. The first cycle I had 3 follies whch ended with a c/p and the second cycle I had 4 follies and that resulted in one baby. We do not have MFI. I think it really depends on your history. If this is your first inject cycle it's hard to know how things will go. I wasn't nervous with Femara and 4 follies since I've had many BFN's with 3 follies. Do you have PCOS? In my case I do, and I think that is why I need more follies since not all of them may contain a mature, viable egg.
In your situation I would move forward with 3 follies considering you have MFI and previous BFN's.

My husband has borderline count and motility and 0% morph and I've gotten pregnant once out of four IUIs, once with IVF and also four times without any medical intervention so having that many follies would make me nervous. Even with our last IUIs, where I was 39, I was worried to have more than two because I really didn't want to be pregnant with twins and I sure as heck didn't want to risk a triplet plus pregnancy.
It's one thing to say you're open to selective reduction when it's in the abstract, it's another thing completely to actually have to make that decision once you are pregnant and have seen ultrasounds.
A 1% chance is really slim so depending on your past history, I might risk it, but at the same time, I seem to fall on the weird side of statistics so then again, maybe I wouldn't! Sorry, I'm not of much help.
Kelly, Mom to Christopher Shannon 9.27.06, Catherine Quinn 2.24.09, Trey Barton lost on 12.28.09, Therese Barton lost on 6.10.10, Joseph Sullivan 7.23.11, and our latest, Victoria Maren 11.15.12
Secondary infertility success with IVF, then two losses, one at 14 weeks and one at 10 weeks, then success with IUI and then just pure, crazy luck. Expecting our fifth in May as the result of a FET.
This Cluttered Life
I have never been in your exact situation, but 1 percen seems really low. I would probably risk it.
With respect to SR, it is a very personal decision, but should the situation arise, I think that should be a decision you make on your own and not being pressured by an RE or another doc. I'm sure they are scared of a possible lawsuit.
With respect to the entire "SR" issue, I don't personally think I could ever SR triplets. (Not judging in the least anyone who has or is thinking about it.) Clearly, I have not been in that situation, so I can't say "never," but I feel like I have just seen alot of women go on to have very healthy babies with triplets. Plus, I think that since I have lived through a high risk/pre-e pregnancy complete with being on hospital bedrest and having a a LO with IUGR and coming out the other end with everything being "ok," I would tend to be positive that even if they are born early or are small that ultimately everything will most likely be okay. There is so much technology now. It is truly incredible. I think back just to when my Mother had my brothaters (twins) in the 60's and she didn't even know she was pregnant with twins until her 8th month!!! The doctor even had put her on amphetamine diet pills (which are now illegal even when you aren't pregnant) because she was gaining so much weight. So my poor twin brothers were exposed to amphetamines for 2 months in utero! We truly have come so far!!
