I'm planning to ask how many cycles of meds and TI they do before moving on to IUI...and a few other questions I've written down.

BFP #1: 2/15/12-Ectopic/Mtx at 6 wks
BFP #2: 10/12/12-m/c at 5 wks.
Me: MTHFR, Low AMH (1.1), High NK cells and Antiphospholipid Antibodies.
IVF with ICSI in August 2013 brought us our babies. ER-9R, 7M, 4F w/ICSI ET of 2-Grade 2 blasts.
Dx shortened cervix, PTL, and preeclampsia during pregnancy. Lots of medications and 13 weeks of bedrest, babies were born healthy at 34w4d!
I'd ask about what your probability of success is with each type of treatment, considering your diagnosis. This can be useful when deciding when it's time to move on to other treatments.
You may also want to ask some financial questions about their policies on payment due dates and potential insurance coverage.

I think everyone pretty much covered it, but I think that a lot of questions will really depend on your diagnosis. For me, with an Unexplained IF diagnosis, my RE didn't really believe it was worthwhile to do medicated cycles without IUI, based on the overall odds of each. So I was able to skip right to the questions about effectiveness of IUI w/our diagnosis, how many tries before moving on, types of meds, types of monitoring, cost, etc.
Also, while I had done all of my testing previously with my OB, my RE did do an internal u/s and did the infectious disease blood panel at my first appt. (5 vials or so). So just be prepared for that as well. Also, bring any charts you have. For me, it was helpful to have a years worth of charts showing that I ovulated and had good timing.

2/1/13- HPT BFP but Beta Hcg was negative
Plan: Femara + Trigger + IUI in April if no real BFP before then Bloggy Blog!
