Combating flat head

So DS has his 4 month check up a week from Friday and I am more afraid the doc is going to say his head is too flat than anything else. DS goes to a nanny share with a little boy who is 14 months and I wonder if the nanny is not doing as much tummy time as we asked because the other little boy is so active.

How do I approach her about this tactfully while still ensuring that she has time for both boys? I want to make sure DS is getting his needs met, but I also recognize she is watching a fully mobile toddler too.
