Intro...Incompetant Cervix & Possible GD

Hello Ladies,

I have been wanting to type an intro on this board for a long time but havent done it because i wanted to be further in my pregnancy.  Let me tell you a little about myself and my pregnancy journey. I have PCOS,was dx when i was 24, I am currently 30. I conceived in 2011 but ended in mc at 8wks, conceived my son in 2012 but loss in at 21 weeks due to IC. Now here I am 11.2wks.

My highrisk Dr, wants to place a cerclage this pregnancy, start 17p shots 16-36wks, bi-weekly via vaginal cervix check and bedrest if needed.

 Yesterday I had an appt and I left in a ball of tears because I was expecting to get my nt scan and schd my cerclage for next week but instead, I was told i was too early for my nt scan, I cant schd my cerclage till I get the scan done and I failed my 1hr glucose test by 1 point. When you struggle to conceive you just pray the pregnancy isnt difficult... Best thing i got out of my visit was i was able to see my little one dancing in my belly.

Now what i have to look forward to is, May 10th nt scan and 3hr glucose test (which i have not idea how i will do that) the 1 hr almost made me vomit all over the place. Then on the 16th ill schd my cerclage which will more then likely be the following week. Memorial Day weekend :0/ (ill be 14 weeks)

I just would like some tips on keeping my sugar down so i dont fail that 3hr test? and ladies with cerclage what do i have in store for me, for after the procedure? What can I expect?

    There's not much you can or should do to try and pass the 3 hour GTT, so don't sweat it--just follow the doctor's directions for what to do prior to the test.  I'm 37yrs, have PCOS, and was diagnosed with GD at 24 weeks--it's really not that bad, so don't sweat it until you know you need to, and if you need to, come back here and the other GD ladies will have TONS of helpful advice for you.  If you only failed the 1 hour by 1 point, then there's a very good chance that even if you fail the 3 hour, you'll be able to control the GD well, maybe even by just making minor diet changes.

    I have no advice on the cerclage, just fingers crossed for you that it goes well!

    congratulations on your pregnancy!  I can help some with the cerclage and weekly shots.  my cerclage was placed between 16 and 17 weeks.  the type of cerclage I got took longer to place (about an hour and half).  I was given a spinal because I was told it would be better for the baby.  so I was awake the whole time.  I've heard many women say they weren't in too much pain afterwards, but once the spinal wore off for me, I was in more pain than I was expecting.  it took a good 2-3 days for the pain to subside where I could take tylenol instead of percocet.  I had bleeding/spotting for about a week, and lots of pressure.  but I think this was all from the type of cerclage I have, possibly my age, and possibly getting it a little later in pregnancy than most.  I had biweekly cervical checks until 26 weeks (by the way, the first cervical check made me bleed for a few hours.  I was told it was because it irritated me, and the doc wasn't concerned).  the shots aren't so bad, mine go in my bum, and we rotate sides so I don't get sore.  haven't had any issues with those other than they get itchy.

    Thanks sweety, i am super excited to get the cerclage over with. Are on bedrest? Did you need bedrest after the procedure?  

  • So sorry you are going through all that! Lots of women on the august board are going through the GD tests right now. Seems there is no rhyme nor reason as to who fails and who passes. So don't beat yourself up too much on that. 

    I had a cerclage placed at 20 weeks. We just by chance caught a short cervix of 1.7 at my 20 week A/S (only found it b/c i requested a vaginal u/s to check into an old cyst on my ovary). I had a cerclage placed the next day. The surgery itself was pretty quick. The prep took longer than the actual surgery! I went in at 9am and was out by 5pm. The pain meds were via a spinal, which I hadn't had before. I requested the cocktail that semi-knocks you out before, so that I wouldn't really remember the procedure. Not sure what it was called... the anethesiolgist called it a margarita IV. :) My recovery was pretty decent - took me a few hours to feel my legs. They said once I could stand on my own and urinate on my own they'd let me go home. I took 1/2 pain pills for the first 36 hours and then didnt need any further pain meds. Frankly, I probably could have gotten away w/o the pain meds at all. My spotting was minimal but it was expected. 

    They didn't put me on bed rest but asked that I greatly reduce my activity. I had been running, hiking, snowshoeing, etc. and I had to stop all that. I was allowed to keep power walking, swimming and light yoga. I continued to go to work as well.

    I've been having bi-weekly cerclage checks and have my last one coming up at 26 weeks. It measured 3.3 and 3.5 until the next issue came about. 

    Unrelated to the cerclage I contracted some random GI virus three weeks ago and spent three nights in the hospital. I went into the ER with severe abdominal cramping and they had to test out EVERYTHING, including doing an Amnio to rule out infection in the uterus. They knew I had an infection somewhere b/c my white blood cell count was super elevated. Between the mild contractions I was already having and the stronger contractions caused by the Amnio, they were greatly concerned re: pre-term labor. My cerclage held strong at the placement point of 1.7. My doctor said she wished it held higher but holding at the placement point (ie. what the cervix measured when they put it in) is common and what they'd expect to see. After this ordeal they put me on bed rest for five days and now just have me on reduced mobility. I work from home, can take a leisurely walk around the block with the dog, and go to dinner, etc. Can't lift anything more than 10 lbs, and am required to get horizontal if I feel any cramping, tightness, etc. The issue with bed rest is that there are no studies that show its helpful and sometimes cause more stress. That said, many doctors still prescribe it (mine included) and I say do whatever your doc recommends!

    My doctor said that most people doing the 3 hr test think they should eat less the night before, but you really should load up on carbs because they absorb the blood sugar. I failed the 1 hr. I and the doctor were both shocked. The 3 hr one was perfectly normal. It was hard to drink it all, but it was almost worse to get blood drawn 4 times. I made them switch to the other arm after 3 times.

  • Previous high-risk and lurking. I am sorry for your loss in the past and want to wish you the best of luck with the rest of this pregnancy!! I was dx with IC at 20 weeks pregnant, thinned to 1.5cm and had an emergent cerclage done. I was given a spinal block for the surgery. The surgery was over in about 10 min and I didn't feel a thing. They had my legs way up in the air, when the surgery was over and they put my legs down it made my blood pressure, which already runs low, drop. That was the only scary part. It was done as outpaitent so I was able to go home a few hours after surgery. They monitored my baby the whole time so that was a relief. My doctor told me I could return to work in 2 days but I started having complications and ended up on bedrest until I delivered him at 35 weeks. The removal of the cerclage was painful for me, though my water had already broke and I was 2 cm dilated with my cervix starting to tear so that may have been the cause of the pain. I don't have any advice as far as the GD but I hope you pass the 3 hour test! T&P for you!

    The doc didn't say I was officially on bedrest, but I was in bed for those 23 days. I was very very sore. I've been on modified bedrest since 22 weeks, though, because I was having contractions I thought they were BH but weren't. I don't think the cx were related to the cerclage however. I'm not officially on besrest any longer, just need to pay close attention to how I'm feeling. I was asked to take it really easy, so I really only do some light prenatal yoga stretches. No running, walking, hiking...I will after the baby! Hope that helps.
