I let DS take a pillow in his crib for naps around 14 months, I think. And I think we started putting it in his crib overnight around maybe 18 months? It may have been earlier than that; I don't remember when exactly.
DD has 2 small pillows that came with her bed set in her crib. She doesn't lay on them. They just sit up against the side of the crib. She tosses them around all of the time so I'm not worried about them.

My older DD had a pillow from 16 mos and used it every night. I gave my younger DD one at 17 mos, and she ignored it, so I took it out of the crib. Depends on the kid.

We added a pillow a around 13 months when he had a cold and have just left it. He sleeps better with it and it is never in a different spot when he wakes up. It is a flatter, older full sized pillow.

We started using a pillow around 13 months when he had a bad
cold/congestion to help elevate him even more since he was waking
himself up with his coughing. And we just never removed it.
We
use the one that came with his activity gym/mat
