Only when I know she'll be outside for a while. If she's at the babysitter's house, I trust that they will put it on her before spending too much time outside.
Up until today, it was winter jacket weather, so it hadn't been an issue. It's finally warm out, and I apply it if we will be outside more than 5-10 minutes.
I'm terrible at remembering to put it on her. I think that if I start putting it on like lotion in the morning it would become a habit and then I wouldnt forget but I'm cautious about rubbing chemicals on her everyday.

now that it is getting warmer Daycare will ask that we apply it in the morning and they'll reapply after lunch. They'll spend a lot of time outside.
I do it daily. Before I dress her I lather her up. On weekends I reapply if we go outside for any extended period of time.
She's super pale and it is super hot and sunny already, so I'm not taking any chances!
