I'd say do whatever your baby is comfortable with. Many little ones will colapse the nipple on slower flows because they are sucking too hard. Others may gulp more air and spit up more because they are sucking too hard. If these aren't happening to your LO then there really isn't any reason to move up to a faster flow unless they are taking forever to eat and you want it to take less time.

I would read what the company that makes the nipple recommends. I use Nuk, and we're still on slow flow (but it is for 0-6 months, which is nice), and DS is showing no signs of wanting to change, although if you are using rice cereal in the bottles, I think using a faster nipple will be helpful.
When we used the Avent bottles, he would collapse the nipple, and then get annoyed that it wasn't getting into his mouth fast enough. Avent has a different age span for their nipples (newborn, 1 month, 3 month, etc.), so we're not going to use them anymore.

Re: Slow flow to medium flow nipples
DS switched to the fast nipples at like 2-3 months.
DD2 is 7 months and still on the slow nipples. She can't handle the flow of the faster ones. She spits up and wiggles a lot more on the fast.
It just depends on the baby.
