Secondary IF

What would 2IF do?

Sorry I've been kinda MIA but I took this second failed IVF cycle pretty hard. Anyway my RE called me over the weekend with our next options. He is really frustrated and confused as to why we are having a harder time getting pregnant this time around and he basically gave me two options. 

Option 1: I have an endometrial biopsy first and then we try another round of IVF.

Or

Option 2: Laproscopy (nothing was found during my Lap before I got pregnant with DD but I had a CS with DD so he's concerned about scar tissue) and then we try IUI with injectables again.

I have no idea what to do. I have 2 IUI and 2 IVF cycles (lifetime max) left that will be covered by insurance. What would you do?

  • I'm sorry that you are having a hard time. If it were me, I would just do another round of IVF. The chances are so much higher and you are very lucky to have insurance that covers it.
    TTC #3 (#1 for DH) since September 2011. DX: Unexplained infertility with possible cervical factor. May 2012 did clomid with IUI - BFN August 2012 IUI #2 w/ follistim/letrozole - BFN September 2012 IUI #3 w/ follistim/letrozole - BFN October 2012 IUI #4 w/ follistim/letrozole - BFN November/December 2012 IVF/ICSI #1 Retrieved 12 eggs, 8 fertilized. Transferred 1 beautiful embryo and 5 made it to freeze. Got my BFP! January 2013 u/s showed empty gestational sac. FET cycle April 2013.  Transferred 2 5 day embryos.  BFP!!!!!  2 heartbeats!!!!  EDD:  12/27/13

  • Hard call! Although I have not had either IUI or IVF, so i have no real experience to base my thoughts on, i also would be leaning toward IVF since your insurance covers it. Having scar tissue wouldn't affect that, even if you do have it, and like already said there is a much higher success rate of IVF vs IUI. Good luck with your decision!

  • I personally would try #1.  We keep having m/c's.  We had a long talk with my RE and we asked about scar tissue from my c-section.  He said it would be very rare that scar tissue would be the cause.

    Can you try option #1 since it's less invasive, see what they find and and then #2?

    i agree with Lauren, this is prob what we would do in your shoes.  We have no IF coverage, so IVF#3 wasn't in the cards for us for a long time.   

    TTC#1: 14 months on our own (did HSG, b/w, SA);
    BFP on Cycle 14--TWINS! Identical twin boys stillborn at 19wks(1/9/10)
    3 break cycles; took clomid 50mg, BFP #2 Beta #1 35, Beta #2 338!!! Owen was born 2/11/11! 
    TTC#2: 4 cycles on clomid: BFNs
    BFP #3: Cycle #5 100mg clomid; beta #1 21; beta #2 6=CP 
    Cycle #6 break cycle TTC no meds=BFN

  • Thanks everyone. I'm leaning towards option one as well but the idea of the endometrial biopsy/scraping the lining of my uterus while in the exam room kinda freaks me out a bit. Has anyone ever had this done? Thanks ladies you're the best.

  • I have not had this done, but I am sure they use some sort of anesthetic.  I had a surprise (by surprise, I mean that's not what my appointment is for) D&C last month with no warning and it really was over before I knew it.  The doctor talked me through it and I just kept thinking of DD and I held onto my phone for dear life.  I needed something to hold on to.

  • I'm not sure what I would do besides use your awesome insurance up! I would keep doing IVF. They can select the best eggs to put back in. I know they put great ones back last time. Have you thought about PGS? I think that is the name for when they test the embies for genetic issues.

    I had an endometrial biopsy at the end of January. It wasn't bad *at all* Mine was not at all like a D&C- they used a pipette to suck some endometrial lining out- took less than 5 minutes, just took a few pieces of endometrium for the lab to look at for abnormal growth changes. 

    They *might* use a curette to scrape in your case. If they go that route they will absolutely use anesthetic/twilight sedation. 


    No sedative/anesthetic for me though- taking motrin or advil beforehand would have been smart. By the time I got home I felt like I had been kicked by a horse... but I was better within 24 hours. 

    Thats what I'm really worried about. I know it sounds silly after all the procedures I've had over the past few years due to either IF or pregnancy. The way my RE made it sound it wasn't so much testing the lining, though I'm sure they'll do that too, as much as it was scraping my uterine lining. Ouch! He said it was shown to help increase implantation/pregnancy rates during the early days of IVF? He also said that it was quick and easy and would be done in the exam room so definitely no sedation.

    All I can think of is my BF telling me how her DH and nurse had to hold her down because of the pain when she had to have retained placenta scraped out after the birth of her DD. I know it's not exactly the same thing or even the same doctor but still makes me hesitant.

    I think I'll call my doctor back and find out a little more about the procedure. Why does everything have to be so darn complicated. Thanks ladies. IF really blows!!!
