I am 18 weeks along and just starting to put together my registry. I have read so many different lists and blogs about what is important and what isn't important and it seems that a lot of it just comes down to individual taste. I will tell you that I am aiming to be a fairly natural mama so I do take into consideration environmental factors when registering for items.
If you had to pick just a few essential things to put on your baby registry that you have found yourself using a lot, what would they be?

honestly, we don't have much so that's really the only things (besides, of course, diapers, wipes, carseat, etc) that I really think are up there with necessary items.
Snuza Hero. Love the peace of mind it gives me.

Fisher price Cradle n Swing (I have the snuggamonkey)
Glass bottles (they heat up quicker and they stay warm longer)
A good soft structured carrier (like an Ergo, Boba, or Beco)
Wipe warmer (Little man hates cold wipes)

1- Something to wear your baby in. ESPECIALLY if you are nursing. Your free hands will appreciate it, trust me, and babies looove to be worn and snuggle!
2- The Pandora lullaby station. Okay this isn't a registry must-have, but maybe you want some speakers for baby's room, because it saved my life early on.
3- A swing.
4- Aden & Anais swaddle blankets. They don't get any better than these.
5- A pack&play. We still use ours all the time at home, and its great when you travel.
Edit- also, do yourself a favor and get a bumbo chair and tray. My DD loved being able to sit up!

If you are planning on CDing, we love Bum Genius 4.0s and Thirsties wipes. I've also used flannel wipes before (homemade) and they are fabulous. I love Planet Wise wetbags (medium for diaper bag and large hanging for home), hemp inserts for overnight, and fleece liners to use with rash cream. We use Rockin Green diaper detergent.

Monitor
Onesie extenders. Even though neither DH nor I is tall, somehow we have super tall, 98th percentile babies. The onesie extenders give us about 2 extra months wear out of our onesies. They're also very handy if you are planning to cloth diaper, as they help get the onesies over the big fluffy bums. We got our extenders through One Step Ahead.com.
Pack and play. We are still using ours regularly.
If you are going to cloth diaper, I cannot recommend Green Mountain Diapers Cloth Eez fitted diapers enough. They aren't printed; they're not super exciting colors. They are basic, plain, cotton diapers, but they are the best diapers I have found. We used them on DS1 before DS2 came along and are now using them for DS2. We use Thirsties covers or Rumparooz covers. Two words when it comes to covers: leg gussets. They keep the messes contained.
Earth Mama Angel Baby diaper rash cream. It expensive, I believe about 15 bucks for a little tub of it, but it's a miracle salve.
Earth Mama Angel Baby body wash and shampoo. Smells like the best oranges you could find.
ItzBeen. It keeps track of how long it has been since baby ate, was changed, woke up, etc. I also use it to keep track of my pumping times. This has been a real lifesaver with DS2. And it's also nice to not have to relay all of that information to DH; he can just pick up the ItzBeen to see.
Boba wrap. A bit on the pricey side, but was worth its weight in gold in the early days!
Sleep sacks.
If you have the chance to do so, make some freezer meals before hand. Super tired, new mom You will thank 35 weeks pregnant You.
If you're planning to breastfeed, get a prescription for All Purpose Nipple Cream. It is a lifesaver. You'll have to get it through a compounding pharmacy and it'll have either 3 or 4 medications in it: usually Mupiricin for antibacterial, Betametazone for steriod, miconazole for anti fungal, and the 4th is a powdered Ibuprofen. If your "recipe" has the Ibuprofen, it will be more expensive than if it were left out entirely. We paid about 40 bucks for my Rx, but it was worth every penny! I recommend having the Ibuprofen in there though. It really does make a difference.
Also if you're breastfeeding, invest in a good pump or plan to rent a hospital grade one. I am renting a Medela Symphony this time around and it.is.amazing.
I preferred a My Breast Friend nursing pillow over the Boppy. Both my boys were big, 8.5 lb plus babies and the MBF offered much more support than the Boppy.

Every baby is different.
My kid HATED his crib and bassinet. So I could have bought one later and not register for the bassinet. But you never know.
The swing was a must! He slept in that

I think you're absolutely right. Every family is different and needs different things for their baby. Have you heard of BabyList yet? They are a universal baby registry that allows you to add any item from any store to one registry. Amazon to independent specialty stores (like eco-friendly ones).
