Basically, we don't know what's going on. His ears were fine, his throat was fine, belly fine, urine negative. She looked in his mouth and said it's not teething related as nothing is irritated and nothing looks like it's at the surface ready to come through.. and the ones that have already come through look good, not red or agitated in anyway..
She did increase his reflux meds as he was about 2mL/day short of what he should be getting based on his weight..
She said maybe it's just a bad coincidence.. he gets a little cold and at the same time his reflux starts really bothering him, so the combination makes him cranky and not sleeping at all.
DH and I took turns last night holding him ALL. NIGHT. LONG. It was rough. But, at least DS slept most of the night.. that's what's important..
Ped said stay off the motrin, and see if his fever spikes again.. if it does, call her and we'll have to do more testing to see what's going on. In the meantime, we're just thinking it's viral

((HUGS)) So sorry you are dealing with that.
It kinda sounds like allergy issues we had when we started supplementing LO with formula. We ended up having to feed him Nutramagin (sp?) and that was $30 a pack and later we were able to get him on soy and gentle. But before we figured it out he screamed & screamed. Luckily, it seems he outgrew the milk allergy. I hope it's not but that may be something to look into.
I'm so sorry you had such a rough night... We had a couple of those when DD was about that age with reflux meds and a virus as the suspected culprits.
You're doing a great job mama! It's hard when they won't sleep on their own...I hope tonight is a better night for you all.

Oh yes, we've been through that before, too.. DS has been on Nutramigen since 6 weeks or so! He also had terrible colic and still does have bad reflux.. DH and I kept saying his behavior reminded us SO much of when he was much younger..
UPDATE:
Last night he slept pretty well.. I added an extra pillow under his mattress on top of the wedge so his crib was on more of an incline.. but, he slept decent. He fought me like crazy going down, but I think that was more because he was so exhausted from not sleeping well the past few days.. He did have a screaming match from 2-3:30, but that was more that he woke up and thought I'd come hold him.. his cry was so different than it had been the other night.. the other night it was a "there's something wrong" cry.. last night it was just that whiny sleepy cry.. the kind that stops the second I approach his room.. so, that was fun, but at least we all got some sleep!!
Also.. his fever seems to be gone.. checked him 2x yesterday when no motrin in his system and it was normal.. I'm really hoping it was just a virus or something that has moved on out.. (and, I'm thinking now I may be getting it!).
Anyway, thanks for all the well wishes everyone!!
