On Friday ? 1/11/2013 I had my weekly BPP and NST with my MFM at 8:30am. Well? after all the scans/tests I always meet with him and review all of the images on a screen in his office. He sat down, calmly talked to me about how I was feeling and then said that he would like to scan me himself again that morning just to check something that might me an anomaly. From there I was back an u/s room and he calmly told me that Baby B was having heart decelerations but he was also having a variable heart rate with accelerations where they like to see them so he felt we needed to devise a plan to move forward. Due to Baby A being oblique, he was very supportive and encouraged an induction for that day since I was 38w3d he didn?t think the risk was worth keeping them baking any longer and asked for me to be back at the hospital with my bags, husband, and self by noon.
I called DH, he come straight home from work, helped me load up all the bags and off we went. I was warned by my on-call midwife that an induction for a FTM can take 3+ days but we would proceed at my direction and I always had an option to accept/refuse an induction step and/or move on to a ?family-centered surgical birth? (aka C-section).
After being admitted to L&D, I had an internal exam that showed that my cervix was closed, 50% effaced, pointed forward, and Baby A was at a -1. They started me on a 12-hour Cervidil suppository to help ripen my cervix in an effort to allow Pitocin to be effective later on and I had to remain on the fetal monitors the entire time and in bed for 2 hours after it was placed. The monitor straps were digging into my back b/c of Baby A?s position his heart could only be found on my left hip bone so they had to create this crazy rigging with multiple straps, wash clothes, etc. just to keep Baby A on his monitor. I sent DH home to sleep knowing Saturday could very well be the day we welcomed our boys and he would need the rest, while I needed to rest as well ? I would only talk to him all night and would probably complain! I ended up staying up all night due to my level of anxiety and anticipation over my boys. Turns out after my first IV of just fluids, Baby B stopped having heart decelerations but due to starting the induction process we couldn?t just go home.
Once the Cervidil was removed in the early morning hours by my L&D night nurse, I was so over the induction process as I made no progress but agreed to move forward with the Pitocin per my midwife and I?s plan as the Cervidil may have ripened my cervix for Pitocin to work not necessarily caused cervical change itself. However, lucky me my body started having its own intense contractions but they were coupling contractions (i.e., back-to-back with no break between 2 contractions) coming every 2 ? 3 minutes and you can only have Pitocin if you have less than 3 contractions in 10 minutes. So they allowed me to have a break from the fetal monitors, shower, and then 7 hours later? my coupling contractions finally slowed to all them to administer my first round of Pitocin around 1pm on Saturday. Because I didn?t sleep Friday night they gave me Nubain and Fentanyl to help me get through the Pitocin with a hope of getting some rest. The drugs did too good of a job at relaxing my body though b/c it caused my natural contractions to slow to every 12 minutes and then my Pitocin contractions started to develop at a good frequency but weren?t intense enough to cause cervical change. After the first round of Pitocin, my internal exam showed I was then 2cm dilated, 50% effaced and not responding to the Pitocin how they had liked so to clear my uterus? receptors for Pitcon, I went on a 2-hour ?Pit Holiday? and they allowed me to shower and sleep off the monitors. By then it was Saturday night around 10pm so I again told DH to go sleep at home and I would take the next round of Pitocin overnight while taking an Ambien at my midwife?s strong suggestion (she asked me to take it Friday night but I didn?t so? she was a little more insistant). I ended up calling DH at 1am in tears telling him how bad the fetal monitor rigging for Baby A made my back feel like it was about to break and it was cutting off my circulation at my hip bones, or at least it felt that way. On the phone we agreed that if the midwife?s morning internal exam after the 2nd round of Pitocin revealed no progress we would discuss the C-section option for Sunday. Again, the internal revealed I was still only 2cm dilated, 50% effaced, and Baby A was now at a -2 so disengaging from the induction process. My options were left with either breaking Baby A?s water and starting the clock towards a C-section due to possibility of infection or just moving on to a C-section. DH and I differed in opinion as to the best option based on the induction process but ultimately around 10:30am we came together to decide it was time to move forward with a C-section without posing any risks of infection to the boys by breaking my water. The stopped the Pitocin, made me stay on the monitors for an additional 30 minutes, and then Anesthesia was knocking at my L&D door.
I was terrified of having a C-section because my big thing for my birth experience was NOT to have an epidural as I was just terrified to have the needle in my back? just the idea still makes me nauseas to think about! To my surprise, it really wasn?t as bad as I built it up in my head and almost as soon as they had it placed, they had me lay down on the OR table very quickly? they asked my drape level preference, left my arms untied, brought in DH and off it went!
On Sunday ? January 13, 2013 we welcomed Trent James at 12:42pm weighing 7lb 0.5oz and measuring 20 inches long followed quickly there after by his brother Ward Vincent at 12:44pm weighing 7lb 2 oz and measuring 21 inches long.
I unfortunately had a seizure post-op around 5pm Sunday which later my labs revealed that I lost a TON of blood due to my slight anemia that apparently wasn?t as slight as we thought and I ended up having to have a few extra steps in my recovery process, so it has been a very rough few days for me personally but I?m so glad that we are all home safe and sound. Both boys have stayed with us since the minute they were born and thankfully were able to fully avoid the NICU as we are now home. They are both doing so well and working on breastfeeding like rockstars! Below is a pic of them post-op sharing a bassinet:
