Vaginal Birth Question

Just curious about what to expect with a possible vaginal delivery...

My Childbirth for Multiples class instructor told us it would be baby-baby-placenta-placenta (or just placenta if they fused or are monochorionic).  That being said, Baby A's cord is just going to be literally hanging out while you push Baby B out.

If your twins make it to full term, meaning their head circumferences are ~ 9cm+  and then add the cord - are you more likely to tear? 

Did anyone experience additional difficulty delivering Baby B due to Baby A's cord?

Thinking about the logistics of it all is starting to worry me since my Baby A's estimated head measurement was ~ 9.3cm at my growth u/s on Friday Indifferent and they aren't seeming to make a move for the exit anytime soon from the feel or looks of things.

  • Just their head size will not for sure cause you to tear, and the cord is  squishy and will likely collapse all the way anyways. how fast they pass through the birth canal and other factors play a roll.

  • The plates in the scalp are not fused making them pliable for birth.

    The cord is not solid and can easily be 'smooshed' to the side and not make a difference.

  • Yep, it was baby, baby, and then fused placentas for me. There were no issues with A's cord that I was aware of. Baby A came out, the OB did a u/s and discovered B had flipped to breech yet again so he did a breech extraction, and then delivered the placenta. I did end up with a second-degree tear but I'm not sure if that was caused by delivering A or the breech extraction of B. Either way, it wasn't a big deal and I have no regrets.

  • My boys shared a placenta, so the cord must have been hanging out - I had no idea during the delivery.  My second baby was very small but he did the tearing - he came out REALLY fast.  My tear was minor, though, and not a big deal.  I would do a vaginal birth over again in a heartbeat.

  • I delivered at 40w0d. Mine was baby, baby, fused placenta.  I had an episiotomy though because I had started to tear and the midwife thought it was best.  Especially since I had been in labor for 3 days and had been pushing for 3.5 hrs.  Still totally worth having a vaginal delivery.

    No. I didn't tear but had to have an episiotomy and wound up with a 2nd degree tear/cut because Baby B turned face up after Baby A was delivered. If she had been face down I'm sure I wouldn't have torn or needed an epi since her head was smaller than Baby A's.

    Did anyone experience additional difficulty delivering Baby B due to Baby A's cord? Nope. It was weird though because I could feel it there!

I wouldn't worry about that. Those measurements can be way off one way or the other. My whole PG they told me Baby B was much bigger than Baby A but Baby A was an inch longer and 5oz heavier than Baby B.

    I was wondering if I would ever go into labor as well. Baby A was head down from 18w on and my OB could feel his head at my cervix checks starting around 30w but I never dilated more than 1cm by the time I went in for my induction at 38w1d (delivered 38w2d).

  • My twins were 8 lbs and 8.5 lbs and I only needed one stitch for my tear.  Granted, it was not my first delivery, but you can deliver a very big baby with little to no tearing (I did; my second was 10 lbs 15.7 oz); likewise, you can have a tiny baby and experience a traumatic injury.
