Subchorionic Hematoma and Update

Good Evening Ladies,

I posted about a week ago about my first u/s appt. at 6 weeks where they found 1 healthy baby, one empty sac, and another odd sac.  My ob has older equipment, so they sent me to maternal fetal medicine yesterday for a better u/s at 7 weeks 5 days.  Baby looked good and measured 7 weeks 3 days with a hb of 153.  We spoke with that doc and she was very optimistic.  Said baby looked healthy, and we have one vanishing twin and a bleed.  Gave odds of 85% chance this works out (15% of miscarriage).  I thought the bleed was rather small, but my ob said it's of moderate size.  Given my history and the fact that I was quite on edge, she offered to have me come back in 2 weeks and then 2 weeks later i'll have my NT scan. 

I'm VERY nervous! I have seen that some of you have experience with subchorionic hematomas.  Please share your experience!  Also, any restrictions.  My ob today (there are many in the practice) said no real restrictions.  I reminded her that my job as a pediatric physical therapist requires me to do LOTS of lifting and moving.  She was not concerned...granted she also told a friend of mine who was pregnant with twins that she should have an MRI of her back (she was in pain).  Even though she knew it was risky?  What gives...

  • Great news!!! That is an awesome update.  I had a SCH with DD.  It was found around 10 weeks.  It was when I first started spotting.  I then continued to spot the entire pregnancy, but the SCH was never mentioned past my 12 week ultrasound.  (By the end of the pregnancy my OB finally decided that I probably had a very sensitive cervix.  But I had lots of u/s during my pregnancy due to the spotting).  I was given a few restrictions for the SCH - basically pelvic rest (no sex, nothing in the vagina).  I was also told I couldn't exercise until I was 72 hours without any blood.  When the spotting continued (which was no longer thought to be from the SCH), I was also had lifting restrictions of 30 lbs (probably starting around 14 weeks).  I am a nurse and at the time I was working on a orthopedic surgical floor which required heavy (machines, morbid obese patients, etc.).  The OB practice I see is pretty conservative, but they also do not believe in bed rest unless there is a specific high risk situation.  I will also say that I stopped my gym workouts (which were pretty intermittent due to the spotting) in the early 2nd trimester, but I was able to walk and by week 30 I was walking 5 miles/day.  My spotting seemed to always occur after I worked a 12 hour shift and not after exercise, so I just paid close attention to my body. 

    I do not have SCH this time around, but have been experiencing spotting (and more than with DD).  I am on the same restrictions of pelvic rest and this time around I have to wait a week without bleeding before I can exercise (I think this is just due to a slight difference in opinion between OBs within in the same practice that I saw).  I do not have a job that requires lifting, so that is not an issue.  The only thing I consistently lift and carry is my 30 lb 2 year old.

    Edit:  When I said "great news" and "that is an awesome update" I was referring to the baby growing/doing well.  I remember your last post when you were worried about size differences and I was excited (for you) that things turned out positive.  I am very sorry that you lost the other baby/twin.  I also think it is a normal response to be nervous about this all.  I hope I didn't come across as insensitive, I think my pregnancy insomnia last night did not help me with thinking clearly.

  • I had a small SCH that was found at my dating u/s (around 6 weeks).  My OB told me I might experience some bleeding and to let them know if I did and they'd bring me in to check it out.  As far as restrictions, I was on light pelvic rest.  No heavy exercise, no intercourse, and no lifting over 25lbs. 

    Around 10 weeks, my SCH expelled, causing some bleeding.  It was really similar in timing to my loss, so we ended up going to the ER to get checked and everything was fine. 

    If yours is of "moderate" size, I'm really surprised she didn't put you on any restrictions. 

  • I had an SCH. It was found at 8 weeks after I also lost a twin.  It stuck around for quite some time, causing daily spotting and one major red bleed.  It was gone by 20 weeks.  I am still being monitored closely though, because I am still spotting every day.

    The good news is, the vast majority of SCHs do resolve on their own.  :). Drink lots of water, stay off your feet when you can. Lying on your left side is ideal.  My doctor did not recommend bed rest, as there is no evidence that it helps.  I was put on pelvic rest (no sex, no orgasm, nothing in the vagina), and told to avoid lifting/pushing/pulling/bending too much.  I took it very easy and am glad that I did.

     If your job involves a lot of heavy lifting, I would think about getting a second opinion on at least that issue.  Will your employer accommodate you?

     I know it's scary, but try not to worry.  The odds really are in your favour. :). Good luck!

    Lol. You did seem a little excited for some mixed news. I understand what you meant though. Thanks for sharing your experiences! I really hope this works out, because I don't know if we can go through all of this again!

  • 1. I'm glad the baby is doing well. That's a great heartbeat!

    2. I'm sorry you have an SCH.  I don't have any experience with that, but my RE said no heavy lifting (no more than 15-20 pounds) and no intense exercise, pelvic rest, etc. She said all of this just because of my loss history (and I'm on blood thinners). I would see if you could get a second opinion on the restrictions.

    Thanks for your response. Those are a lot of coincidences! Where did you go to grad school? I ended up going to University of Delaware. I work in a private school for kids who are multiply disabled. And it just so happens that a majority of the kids on my schedule this year are a dependent lift. Luckily they are 38 year olds so they are 30 to 60 pounds. I'm not sure how to go about getting an opinion about any restrictions. I could always call the md from mfm. Although I don't remember her name.

  • I was diagnosed with one at 7 weeks. I was put on bed rest for a few days. I was also put on pelvic rest and told no heavy lifting. Mine lasted for 6 weeks. I went in for a sono every 2 weeks until it was gone. I had bleeding episodes and spotting episodes. I wish you the best!
