I just got two Enfamil $5 checks in the mail (exp 2/2013), along with 4 samples of the Enfamil Premium Infant pre-packaged to go formula.  I believe they make 4 oz bottles.  I'm looking for Similac checks.  Email me at ashleyeichenlaub at gmail dot com.
