History of 3 cerclages but they told me I do not need one with this preg!

Ladies I lost a 23 week baby 10 years ago and have had 3 cerclages and 3 healthy kids. I am prego with #5 and the new docs say I do not need a cerclage. What? But after talking with them I hope they are right and I have a healty full term little boy. It is hard to believe that I do not need one. Anyone have a cerclage and then not get one with a later pregnancy?

We are doing weekly cervical lengths to watch and if cervix changes then I will get one. They say the new research and the risks say do not do it. I am a little nervous but I will trust in this new high risk group of doctors.

Do you think I am crazy for not getting a 4th cercalge? Had a cervical length today that looked great. They tell me the risks are no different now to place it as later as long as I do not have a buldging bag or contractions so I am hoping with weekly cervical lenghts we will catch any changes early.

They tell me what I had with the loss was preterm labor and not cervical incompatance because I had pain associated with it.

  • Personally, that would make me very uneasy. I had a 22w loss last year. They also thought it was ptl and not ic because I had painful contractions. Because of that, this time around my MFM opted for the wait and see route. I had my cervical length checked weekly. Despite 7w of bed rest, my cervix suddenly shortened from a 2.5 to below a 1 in less than a week at 22w. I'm just lucky it happened when it did, I had an emergency cerclage placed at 22w5d. By the time they placed it I was 8mm and 1cm dilated.

    The risks from an emergent cerclage are much higher. There's a risk of rupture of the membranes as well as infection, particularly if you are dilated at all. I consider myself extremely lucky it worked. Had I not had the cerclage place I think I would've been looking at another 23w delivery.

    My MFM has said that in any future pg I should get a preventative cerclage.

  • Like pp, this would make me nervous.  However, if your dr feels confident that you don't have IC I might feel a little better.  My stance really is what if?  What if you do have IC?  Personally I'd rather be safe than sorry. 

     

     

  • I think it is interesting they don't want to do one. My MFM said once you get a cerclage you need one for subsequent pregnancies. I had no pain at first and then once unstoppable labor started I had pain, so it is a toss up on whether I have IC or not, but my doctor didn't think it was good to do the wait and see approach. But for you my concern would be that they have already done a procedure on your cervix which I assume automatically makes you high risk. It might be a good idea to get a second opinion. Can't hurt.
  • This is a new group of MFM and the old group said once a cerclage always a cerclage. It is hard for me to not make the decision to get one since I have been an baby nurse so I know all to well about PTL and the like.

    Just unsure really what to do. I wish I could get an exact answer of what will happen but no one knows. I asked all the questions if my history of the 3 cerclages weakens my cervix and was told no that the scar tissue can make it stronger.

    Cervical length today was great! I will make them do weekly and perhaps even bi weekly as we get closer to 20 weeks. With my history I seem to have issues around 23-24 weeks.

    I talked with 2 MFM and they both say the same thing about the cerclage.

    I saw y regular OB today and he said he would do a cerclage. But the high riks docs the MFM said no.... What to do. I was told today by my Ob any time he would put one in if I wanted it! Hmmmmm..............I hate making a decision like this. I had a cerclage scheduled for yesterday but cancled it.

    Something tells me everything is going to be ok and I do not need one. I went into this preg thinking do I really need one. We shall see...

  • I don't understand your dr's reasoning? If you had three successful pregnancies with one and a loss without why take the risk? The risks with a preventative cerclage are rare and minimal. To me it was a simple decision and I'm seeing one of the top high risk dr's for cerclages in the world and he thought it would be stupid to do the wait and see approach.... Especially since I have a toddler at home.

    Even with a great measurement now it means absolutely nothing because the cervix can change fast. If you really don't feel you need it then request extra close monitoring and prepare for bedrest just in case. For me it wa about piece of mind and being able to care for my other child. I did not want to wait for things to get bad before doing anything
  • I'd get the preventative cerclage too. I couldn't see taking the risk of not having one and having a rapid cervical change later, and also the increased liklihood of bedrest.
