This might be a silly question but i have to ask. I am nursing in 4 hour blocks. What happens if my DS sleeps for a 4 hour stretch? So if he last ate on left side, sleeps for 4 hours, do I start with the right when he wakes or skip the right side for the next 4 hours?

When block feeding, I went by number of feedings instead of the clock. So, I would feed from the left for 2 or 3 feedings, then I'd feed from the right for 2 or 3 feedings. It was easier for me to be consistent that way, and not worry about different scenarios.
But to answer your question, if you fed from the left, then LO slept for 4 hrs, you'd feed from the right next.
