Requesting Operative report

I saw my OB today (I'm 11 weeks) and he said he wants my operative report from my scheduled c-section with my daughter.  Do I call my old OB or the hospital I delivered at?

  • I called the hospital directly- there was an extension for records.  I believe I had to fill out request paperwork to get it from them.  It was free if I had it sent directly to my new midwife, otherwise I was going to have to pay for it.  So you might want to check if there is a cost.

  • I got mine from my previous doctor. They just faxed it right to my new doctor.

  • I contacted the hospital's records department, but if you call the office directly, they can either release them or point you in the right direction.

    I called the hospital since it's part of your hospital medical record.  It's possible that your MD might have a copy too, but the hospital should definitely have it.
