Survey help please?

Hi! I usually only post on my BMB, but am looking for some help with a school project. I'm taking a Research Design class, based on social media.  We're required to do a social research project, and I've chosen to do online communities and forums, since I spend a lot of time on this community I though it fitting. One of the requirements is to do a survey, and I was hoping to get some help with that.
