Growth discrepancy? (di/di)

Hi all, 

I have di/di twins so not a TTTS question. But I am concerned about the size difference. At last MFM appt, Baby B measured 21% bigger than Baby A. Baby B has always been a bit bigger, by a few days or so, but not this different.

However, the MFM didn't appear very concerned, because Baby A was still on track and measured in the 27th percentile (B measured 89th percentile). She said if A were too small, then they would be concerned about cord flow, growth, etc, esp over 20% difference. But since A was still in normal range (B is just a giant) that it isn't that big of a deal. She also said it is more likely that they are fraternal with different growth rates. I was worried because baby A initially had a marginal cord insertion, but she said it has migrated to be more central and she isn't worried.

But of course I'm a little worried. Did any of you have such a big difference in your twins, and did it turn out ok? Or not? (They have fused placentas, too). I remember someone on this board with rare TTTS in fraternals lately, I know it probably isn't that because that is so rare. But I would appreciate some reassurance from others' experiences! Thanks!

  • I'm about a week behind you. We have a 17% difference with our baby A being the bigger of the two. I was told that our boy (A) is measuring big while our girl (B) is measuring small-medium. Like yours, our MFM was not concerned with the difference saying that they are both still measuring on track and just growing at different rates (di/di boy/girl twins here). I know you have a slightly bigger size difference, but I think it sounds OK based off what your MFM said and the fact that mine said something similar.

  • We had a growth discordance but was told 15-20% was to be watched but not worrisome. At 32 wks A was 4.10 and B was 5.9. At 34wks A was 6.2 and B was 6.6 and 36wks A was 6.4 and B was 7.5. I delivered at 37w1d A was 7.3 and B was 7.9. HTH

  • Mine were 4.5 and 6.3 at birth, fraternal twin boys. At 3 months, they are just over half a pound apart. Mine were delivered at 36 weeks because of the growth discrepency. I would say if MFM isn't worried, I wouldn't worry either.
    Mom to 2 beautiful girls, 3 yrs and 22 months old. My 2nd was born at 32 weeks due to Rhogam failure/severe complications from Rh disease and is our miracle. She has bilateral auditory neuropathy and a cochlear implant, activated 4/5/2012 at 19 months. Lilypie First Birthday tickers
  • We actually have a mom on this board who's di/di twins had TTTS.  My boys FINALLY has caught up with each other when it comes to their growth.  I think they are maybe a day behind.  It took awhile for them to get there. Growth changes so rapidly.  If the MFM isn't concerned I wouldn't worry to much.

  • I normally just lurk, but I had to comment because we are dealing with this exact same thing right now. I have di/di b/g twins and at last weeks u/s baby A was 4lb 12oz and baby b was 5lb 11oz. I had another u/s today with a MFM (1st time going) and A is now 5lb and B is 6lb 8oz. She said that everything is probably fine, but that I should continue go for my non-stress tests so they are constantly monitored. Have you started that yet? I know I started at 32 weeks, so maybe that's the next step for you. My dr. also said to increase my rest and lay on my left side any chance I get. Anyway, I don't know if that helps, but I just thought I'd share :) Good luck!

  • I was put on bed rest for this @ 30 weeks. My OB wasn't too worried, but said bed rest was standard for 20% + discordance. Dd was always in the 30th percentile & ds w always 70-80th percentile. They were 5 lbs & 6lbs 9oz at birth. Ds is now over 3 lbs bigger! They were in the 25th & 75th percentiles at their 4 mo appt. :)

  • Yeah. My di/di twins had TTTS. It did not start to happen till around 28 weeks & every week it got more & more extreme. They were right on track up until this point. The placentas were fused together & caused her's to stop growing. She was in the 50%, then 40% etc... till we got into 3%. He was in the 50% then 60% 85% etc....They kept telling us that our son was just a big guy & not to worry since they were di/di. But then after while they said that it does happen & is rare but does happen.

    Doesn't sound like you are there yet, but could be with the fused placentas. Good luck.

     Her cord blood flow was the factor that they took into account the most & as soon as it slowed we went in for a c-section that day. They were 4 lbs 3oz apart at birth She is growing at a faster rate now. He lost a lot since he wasn't sussposed to be that big. She is 5.3 lbs & he is 8.7.

  • My girls had a 24% size disparity. They still about a pound apart, but now that they are 15 months old, the difference isn't so big.
  • OK, well thanks everyone and we will just continue to trust the MFM. Maybe we will get more frequent monitoring if it continues.

  • At 32 weeks, the discordance between my twins was greater than 20% so I started more frequent monitoring. I had NSTs twice a week and every other week I had BPP/ growth u/s/ doppler cord flow studies done. By 36 weeks, They were estimated to be about 2 pounds apart and A wasn't growing enough so I had my babies that week. They ended up not being that far apart. A was 5lb 3oz (classified SGA but not as small as they thought) and B was 6lb 5oz (smaller than they thought- he was estimated 6;b11oz). A was fine, B had to go to the NICU for breathing help because he was not stressed in utero so his lungs didn't develop as quickly as A's did from being stressed. GL!

  • My di/di boys had a 21% growth discordance at 32 weeks and again at 36 weeks.  Because they were both bigger than average singletons at that age, there was zero concern.  It was just chalked up to genetic differences.  They were born at 38 weeks exactly and weighed 7lbs10ozs and 7lbs2ozs, no NICU or feeding issues.  

    The only 'issue' the discordance caused in my pregnancy was the fact my dr. Wouldn't do a breech extraction with Baby B being that much bigger than A.  A ended up staying breech, so it didn't matter.

    Good luck growing those babies! Sounds like you are doing a great job!
