I use KiddoTags (Kiddotags.com). I only have the permanent ones for his bottles, but they make clothing labels too. There are lots of different options, and they are cute too!
I ordered labels from mine4sure.com. They have different sets, including 60 clothing labels for $15.
I was just going to use a sharpie, but my penmanship is horrible and sharpie often wears off plastic (bottles) after a few washes. I also figured I'd be more excited about daycare if I used cute labels. I got labels with just our last name so I could use them for either child.
I just label our bottles because it's a small home daycare and use labels from inch bug.com. They've been on for 5+ months so far and no signs of coming off! Was happy with the site and their customer service. They usually have coupon codes out for a % off.

I'll second inchbug.com. Their labels are wonderful. I don't know about labels for coth diapers though. They do have clothing labels made for tags but it specifically says not to apply them directly to clothing.
