IMHO and I think that of others on this board, family surnames are the exception. I think using some random surname that has no significance to you is kind of weird. But it makes the last name as first name meaningful (and would make the whole trend a lot more interesting) when people use family surnames as first names. DD and several of DS's siblings have family surnames as first names, and we considered my maiden name as DS's first but ended up using it as a Mn instead.
But I do think Collins seems pretty masculine, because of its resemblance to Colin. If it were in my family I might use it, but only for a boy.

Call me crazy, but I love it. Just pair a very feminine middle name with it. This is done alot in the South (where I'm from) and I wish I could've done it, but my maiden would never work. My cousin is named after my grandmother's maiden, so his name is Pepper. He totally pulls it off and if Pepper can work, Collins totally can!
I really dislike it. I also think that the need to clarify that it's a maiden name is a bad sign. I don't think your child's name should require an explanation.
I actually really like it. It is a guilty pleasure name for me. I probably wouldn't use it, but since it is a maiden name for you, I would go for it. I am definitely in the minority though!

A lot of children's names go along with an explanation. I think 'how did you decide upon that name' is a pretty routine question. I don't think OP NEEDED to clarify it to the general public, but I think she thought it'd be nice if we knew the background. Her immediate family and friends will obviously already know.
I actually don't dislike it at all! I think its really cute, and thanks to the Blind Side its not too weird. It's got meaning, your other daughter has a unisex name, and I think it's adorable
