Ganirelix instead of Menopur

I have always been lupron, gonal, menopur for IVF's.  I remember one of my RE's saying that if I wanted to cycle with my OE's he would recommend using ganirelix instead of menopur.  It helps with egg quality.  Has anyone else use this in their protocol?
I am AMA and all tests on H came back normal
3 failed IVF with OE and 2 failed frozen DE cycles
Last fresh hail mary DE cycle starting Feb 2013

  • I've seen protocols using low dose antagonist through stims but the two meds are very different. I would not consider Ganirelix a direct replacement since it contains no FSH. Does that make sense?

    GANIRELIX AND CETROTIDE (ANTAGONISTS) 
    INJECTABLE, SUBCUTANEOUS
    These both block the effects of the naturally occurring gonadotropin-releasing hormone (GnRH). GnRH controls the release of another hormone, luteinizing hormone (LH), which induces ovulation (release of an egg from the ovary) during the menstral cycle. During hormone treatment for ovarian stimulation, premature ovulation may lead to eggs that are not suitable for fertilization. Both Ganirelix (premixed syringes) and Cetrotide (vials) are used to prevent premature ovulation during controlled ovarian stimulation.

    menopur 

    It provides follicle stimulating hormone (FSH) and luteinizing hormone (LH) that help healthy ovaries to make eggs. This medication is usually used in combination with another hormone (human chorionic gonadotropin-hCG) to help you become pregnant by bringing about the growth and release of a mature egg (ovulation).

    It's been a long journey. TTC since 9/06. multiple IUI's and IVF's and 4 m/c's. IVF#3 = BFP, twins, induced at 34w6d due to baby b passing away (no explanation). Delivered on 35w1d, Baby A - baby girl, and Baby B - baby boy, our little angel.

  • Yeah, I agree with others.  I used ganerelix in most my cycles to supress but also used Menopur and gonal f as stims.

    That would make more sense.  You typically have one type of ovulation suppressor -  Antagonist cycles use Ganirelix and Agonist cycles use Lupron.

    Probably.  My first cycle they tried lupron and it was a disaster.  After that they used ganerelix but my stims stayed the same with the menopur and gonal f. 

  • Agree with pps, it would be Ganielix instead of Lupron. For issues including poor response, DOR, AMA etc the recommended protocol is usually an antagonist instead of agonist.
    Dx: High FSH, stage IV endo, homozygous C677T MTHFR and PAI-1
    Early loss 10/08
    Lap 1/09
    IVF #1 "natural IVF" - 1 egg retrieved, missed m/c
    Tried several mini-stim cycles with no response
    Switched clinics - dx'd as carrier for Fragile X
    IVF #2 MDL protocol Jan/Feb converted to IUI, BFN
    IVF #2 take 2: Antagonist, one embie, BFN
    IVF #3: Antagonist, no fertilization
    One last ditch effort at OE IVF (antagonist with Clomid) cancelled

    10/28/11 Baby girl lost at 17 weeks due to pre-term labor. We love and miss you.
    DE cycle #3 June/July 2012 - BFP, twins, both heartbeats stopped, D&C
    2 frosties but don't know what's next
    FET Dec 2012: BFP! Praying this one sticks for the long haul!
  • Ganirelix is a sub for Lupron. Not menopur.  Some re's believe Lupron can cause embies to be more fragmented.  So they prefer antagonist w gani vs long Lupron.

  • I'm doing a Ganirelix & Menopur combo this cycle (along with Follistim).  My RE said Gani is usually better for people like me who had an egg issue (which we didn't know about til IVF #1).

    I subbed ganirellix for Lupron for my 3rd fresh cycle, but also used menopure.
