I think it's more the mother's body isn't ready or just won't do what it is supposed to do for some unknown reason.

I think its a little of both. I think when they fail because of failure to progress its because mom's body/baby wasn't ready.

I don't know that you can make a general statement about that. I've been induced twice. The first time was at 34 weeks due to pre-e. I was not dilated or effaced at all to start and had a successful induction. The second time was at 35 weeks due to low amniotic fluid and IUGR. I was dilated to 2 when the induction began, but had a c-section after a few hours because DD was showing signs of stress even though I was progressing. So in my case, the success or failure of the induction had more to do with how baby handled it than whether or not my body was ready.
My mom was literally a month over due so I doubt my brother wasnt ready to be born but I do think there was a miscommunication in her body because even after they induced her and she was a month overdue her labor still didnt start and she had to have him out by c-section.
There's at least one study that showed a decent amount of c/s are due to "failure to be patient" on the part of the provider (in that, women weren't given adequate time to dilate or push before a c/s was called). I don't know if it dealt with induced or spontaneous labors, though.
At any rate, that's another factor to consider, not just the mom and baby.
I think it depends on too many factors to answer this question. What is the reason for the induction? What is the mother's Bishop Score? What kind of induction methods are used? Is the mother allowed to move around and eat? What is the baby's position?That's just what I can think of off the top of my head.

I was induced because DD's heart rate was dropping with contractions and wasn't moving as much as they'd like.
I was induced early on a Saturday and by Monday still hadn't made it past 5 cm. I had pitocin on Sunday, but they had to stop it soon after because it caused a contraction that stopped DD's heart.
I also had cervidil and they broke my water (and found meconium).
Anyway, I know it's been two years, but I still sometimes wonder what went wrong.
In my case, I was induced and an hour after getting Cervadil, my body started contracting on its own. They took it out after 4 hours instead of 12 because I was contracting so well. They did start Pitocin in the morning but I was on a super low dose all the way until 10cm.
So my body was not the problem. I was induced on my due date and my body was ready.
But my baby's head would not fit through the birth canal. I pushed for FOUR hours and they were even gearing up to use the vacuum before my OB realized she was still too high for it.
I'm pretty convinced I would have ended up with a c-section if I had gone into labor on my own. She was never going to fit. I'm very petite and she has a large head.
