Low AMH levels

Hi!  My hubby and I have been TTC x 18 months.  Initial AMH was 1.5, now 1.27.  I've tried multiple rounds on Clomid, then Letrozole now Letrozole with dexamethasone.  Went to a specialist that wanted to jump straight to IVF.  I'm almost 33.  I'm thinking about trying IUI first with my regulary GYN b/c I feel like that's a big jump to skip that when that's a more conservative option.  Any ideas or advice?

  • Your AMH is not terribly low. I am 33 and mine is 0.3, now that is low. Your AFC is a better indicator of how your will respond. Do you know what your AFC amd FSH are? AMH is only one tool used to indicate if one is DOR, but there is more that goes into being DOR than your AMH.

  • Ditto Lynn, but wanted to add that GYNs often don't offer proper monitoring for IUIs or for medicated cycles in general. YMMV but the amount of people on this very board upset over something their GYN did that no RE would do would astound you. Do you have any other issues? IVF is not something that's suggested right away for someone with your AMH unless you have MFI or tubal concerns etc.

    Also, AMH will fluctuate a little and 1.27 and 1.5 are not that far off; < .4 was the point of concern at my first clinic. See: https://www.advancedfertility.com/amh-fertility-test.htm

    I agree with the other ladies.  I would be jumping for joy with that AMH.  Mine unfortunately is <0.2 and I'm 31.   GL with TTC, I wish you the best!

    DE IVF# 1(cycle #6) w/pgd, (freeze all): 30R, 23M, 15F, slow/poor embryo development, 4 biopsied, 1 Normal "Norm"; DE IVF w/PGD, incorporate electro-acupuncture. 
    IVF# 6: (OE/DS) cancelled
    IVF# 7: (OE/DS) 1R, 1M, 1F, arrested day 5
    Plan-DE IVF# 2 (cycle #8): DE/DS in May 2015

  • I think you should try IUI first.  I don't think those AMH levels are all that bad.  My AMH fluctuated between undectable and 0.34, and I am much older than you- the RE still wanted to try IUI (we did 4 rounds) first.

    DOR and AMA
    2/12-5/12: 4 IUI cycles = all BFN;
    7/12: DE IVF # 1 (with ICSI)- 20R, 16M, 14F, 5DT of 2 blasts; 6 frosties = BFN;
    Lupus anticoagulant initially high, then found to be normal on hematology consult;
    Follow up testing in September all clear;
    Started synthroid for "high normal" TSH;
    FET # 1- late October 2012- BFP on FRER; beta # 1- 21(low), beta # 2- 48 (still low), beta # 3- 132, beta # 4- 1,293; beta # 5- 5,606; last beta- over 100,000. First u/s 11/21- heard heartbeat

    Level 2 ultrasound at 20 weeks shows vasa previa and VCI
    Referral to MFM and mandatory c section for delivery

    Finally home after 15 day NICU stay!

    FET #3, early July 2014; beta 7/14, BFN
    DE IVF # 2- August 2014; 14R, 13M, 11F, 5dt of 2 blasts (3 AA), 5 frosties = BFN
    FET #4- December 2014, yet another BFN

    Dr. KK work up shows borderline uterine blood flow, elevated NK cells, and MTHFR mutation (homozygous for c677t)

    Added baby aspirin, prednisone, supplements, Metanx, and intralipids

    Switched to large clinic for final attempt; had endometrial receptivity testing in January; FET March 2015 = yet another BFN

    Likely OAD- NBC
