well...URO update

i guess they didn't tell me over the phone exactly what was abnormal because THERE WAS NOTHING ABNORMAL ehhhh....i know its bad but i was actually relieved that there was something abnormal in hopes that it would help us with our MFI issue! grr...

his fsh was 4.9 which he said was in our favor cause it meant he HAD sperm which makes no sense because his count is only 8 you would think it would be higher since were so close to azoo...

 he did an ultrasound which came back normal as well...

he told us "donor sperm or adoption will prob be your best bet" he wants us to do a tissue biopsy to see if they can get sperm that way and to see if there is a little blockage that they can't see via ultrasound, but he told us not to get our hopes up of them finding the "mother load" as he put it.

since we aren't ready for IVF right now he said we can grasp as straws and he wrote my husband a prescription for clomid to try for a few months to see if we can at least get his count up to something they can work with for IVF...he said do the clomid for a few months then do the biopsy...

but again he didn't order another SA....you would think he would order another SA after the two months of clomid BEFORE going in to biopsy to see if that was helping? idk....

my husband is super discouraged and i feel awful cause i can't help..he said he hates not feeling in control of his own body he said he legit is angry at his body :( 

so i guess clomid we try...ehh I'm ready for some GOOD news already

    If it were my husband, I know he would insist on getting another SA done BEFORE a biopsy. I hope the clomid helps!

    and our MFI uro (one of best in country and invented mTESE) said sperm from SA is always better than sperm that is retrieved.  when you move onto IVF, it would possibly be better for your DH to give multiples samples instead of doing biopsy.

    It's been a long journey. TTC since 9/06. multiple IUI's and IVF's and 4 m/c's. IVF#3 = BFP, twins, induced at 34w6d due to baby b passing away (no explanation). Delivered on 35w1d, Baby A - baby girl, and Baby B - baby boy, our little angel.

  • he is...my husband is such a just go with the flow person he would just do exactly as the doc says without questioning ...me on the other hand I do too much research and common sense would say umm check the count before cutting into him! lol..of course after I put it that way he was like oh yea thats true i guess we will ask after we're done the clomid. so it will get done!

    thanks so much for that info...ill def be talking to them about it and ill also talk to my RE about it which I love.....I think we are going to take all of his tests he had done and switch uro's anyway i just don't get the best vibe from this doc.

    go with your gut!

    i wish i hadn't let our first uro cut into dh's balls.  my mfi guru told us (not outright but in a round about way), he never would have done a biopsy on my dh and a poorly done biopsy can kill off sperm production.  my DH is completely unexplained NOA and our MFI guru could not explain why no sperm was ever found.  

    so again find the best uro you can find and follow your gut instincts

  • I don't know if adoption or donor sperm are on you list of things to try, but it you want to eventually procede with IVF using your husbands sperm then he needs to have a second SA done.  They only need enough sperm for each egg.  If the uro you're seeing won't do that, I'd seriously seek out another uro. 

    Also, if the clinic you worked with won't procede with IVF-ICSI due to such a low count, a drive an hour west of you will - because they did it for us when we got 30ish sperm.

    I hope its ok I post here....

    But please please make sure you do another s/a before surgery..... My DH had an mTESE in November 2011 which was succesful, but the sperm did not survive freeze/thaw. He gave a complete zero count in July 2011 and 200k count in March 2011.  He miraculously had enough to for ICSI in a sample he gave in February 2012. He started supplements and cut out drinking about a month before his surgery so we can't help but wonder if it was unnecessary.  While he recovered quickly, it was a very very emotional and stressful time for us that I know we would rather have not gone through.

    to be completely honest i haven't even talked to my husband about donor sperm I just don't know how I even feel about it i have my concerns about it and he is so sensitive about it right now i don't want to even bring it up yet and we have talked about adoption and I'm totally fine with it but he wants to make sure we have exhausted our other options before which I understand......and I think we are going to go see the Uro that you suggested to me! ...(at least i am going to talk to my husband about it) my gut just says go see someone else, I've had a weird feeling about this doctor from appointment  one!......and thank you your continued thoughts and prayers throughout this means A TON!! :)

    So I clearly read your generic clomid post first. After this, I am very concerned. That was so mean to say DS or adoption are the best bet!  YH had 8, he produces sperm you just need to figure out how to get a few more. And the uro should help with this not just jump to surgery!  I just feel that your uro has this plan but oligospermia/azoo never goes according to plan. There is always a new hoop to jump through and I wish your uro would be more supportive. 

    I am sorry that YH is having a hard time with this. We have all been there so make sure you give him a big hug and let him know how much you love him.

     i am going to call my RE tomorrow....fill them in on whats going on and get his advice as to what i should do, but I think he would def order one if he felt like the uro isn't...but i really think we are going to see another URO...i just talked to my husband about it and he said " ill go wherever you want me to go" i just don't know how many people you need to hear it from that i can't give you a baby :( makes me so sad for him...i did explain to him why i thought we should go see someone else and he said he had no problem with it and I've educated myself more then he has so if he thinks we should then he will make the appointment. I'm going to run it by my RE as well but i have a feeling he is going to agree with me

  • Saw your post on my post :) sorry to hear that we are following similar paths but at the same time it is nice so we can bounce info off each other! I am now a little worried about proceeding with the biopsy. Our uro didn't suggest another SA & I forgot to ask him about it (feel like there are just 100s of questions running through my head!) I will def ask when we see our RE tom. I guess I just thought we'd have to wait until July to give the sperm time to regenerate. So we'll see what RE says but now I am so freaked out that DH may have "maturation arrest" & that would be pretty devastating to our option for IVF/ICSI & we just need to know what is going on so we can make an informed decision
    TTC since June 2011
    DX: DH (30) severe MFI, severely low count & low motility
    Me (32): all clear
    Appt with Urologist 5/21/12: exam, ultrasound, bloodwork all normal.
    Testicular Biopsy with TESE on 6/8/12. good sperm found! (left side only) froze sperm, failed thaw test :(
    Orientation for IVF/ICSI on 6/13/12. Waiting for the green light following biopsy results...results show adequate sperm production both sides.
    2nd SA 6/18/12: sample is "adequate for ICSI"
    Plan: IVF/ICSI July 2012!
    ER: 7/26/12. 15 eggs retrieved, all mature.
    TESE/TESA/aspiration from epididymis, no motile sperm found :(
    froze all eggs, the saga of praying for good sperm continues.
    8/3/12: 2nd opinion from MFI uro on biopsy slides. Suspects "partial late maturation arrest."
    Plan: more SAs, third biopsy/TESE with frozen back-up either from DH or DS.
    SA 8/17/12: Zero sperm
    SA 8/23/12: Zero sperm
    9/26/12: SPERM FOUND! 15 eggs thawed, 12 survived and were ICSI'd, only 3 fertilized normally. Refrozen as embies and will thaw in Nov. Please survive and grow!
    All 3 survived the thaw on 11/15/12!
    FET 11/17/12: transferred 2, one 4B, one 4C. Beta 11/30:BFFN
    moving on to DS
    DIUI#1 2/18/13,50mg Clomid cd 3-7, Ovidrel trigger, Beta 3/4/13: BFN.
    DIUI#2 3/19/13, 50 mg Clomid cd 3-7, Ovidrel trigger, Beta #1 (14dpiui) 4/2/13: BFP!!!! 150. Beta #2 4/4/13: 420 Beta #3 4/8/13: 2691. Beta #4 4/15/13: 15,086
    1st u/s 4/8/13 shows one gestational sac
    2nd u/s 4/15/13 shows yolk sac, fetal pole and early heartbeat
    3rd u/s 4/25/13: measuring right on track. Heart rate 148 bpm

  • awww sweetie - this uro is depressing me!

    get a second opinion for sure. with all the technology out there and TESE and TESA options this uro has a horrible opinion!

    my husband had NOTHING (and i mean nothing)  to deposit at the cryobank, it was all just fluids. yet they obtain a small amount during surgery thanks to the TESE procedure and thanks to IVF and ICSI we still have a chance.

    this urologist needs to get with the times! don't listen to that gibberish!
