Miscarriage/Pregnancy Loss

Low HCG, Low Progesterone

I'm 5 weeks and found out today both HCG and progesterone are low.  HCG is 27 (dr said ideal is at least 50) and Prog is 2.4 and ideal is at least 15.

We are repeating labs on Sunday.

Anyone had similar situation and had the pregnancy continue?

BTW, we have been TTC for over a year and had our first m/c a year ago around this same time.

Re: Low HCG, Low Progesterone

  • I'm sorry you are going through this. I'm also sorry to say that you won't find success stories here. Maybe ask over on 1st tri.

Are you sure of your dates? 27 is not viable if you are, indeed, 5 weeks pregnant, and 2.5 progesterone is not indicative of a normally progressing pregnancy.

((hugs)) I am so sorry that you are possibly going through this again. Hang in there. You will know more on Sunday.

  • I'm sure of dates...we have been charting, had postive surge, etc.
  • I'm sorry you are going through this. Sadly, you won't find success stories here. You might try asking PgAL - that's where you will find a success story if one is to be had.

Did they give you progesterone supplement? The low progesterone is much more dis-concerning, though the HCG is very low for 5 weeks.  With supplements, there may still be a fighting chance for the pregnancy. I will be honest though, with such a low level sounds like a chemical pregnancy

  • Katherine mentioned progesterone supplements. You also mentioned that you have been charting. Do you have short luteal phases? break through bleeding during the luteal phase? If so, then you may suffer from chronic progesterone deficiency in which case you would need a supplement for each of your pregnancies.

If you don't know if you have a LP defect, then maybe your doctor could give you a supplement since there would be no harm in doing so, especially since you are already pregnant. I will be honest, though, that since your level for 5 weeks is way outside of the range of normal, your progesterone level is likely a reflection of that. Progesterone is secreted by the corpus luteum, and is dependent in part on the feedback from the developing pregnancy. In other words, if the pregnancy is not developing normally, then the signal to produce the progesterone is very weak.

With my first miscarriage, I had low hcg and low progesterone. I talked to my perinatologist about supplementing it. My pregnancy was a spontaneous one (not IUI or IVF), so I was not proactively on progesterone supps. She gently explained to me that she would put me on supplementation if I really wanted it, but that she didn't think it would help. I was upset. She explained to me that my luteal phases were nice and long (16 days, which is on the long side), I had never had breakthrough bleeding, no sign of ovulation issues, and history of one successful pregnancy. There was no reason to believe that my low hcg and progesterone was due to anything other than a chromosomally unhealthy pregnancy. I was devastated. And concerned for my future pregnancies. What if they were wrong and I needed progesterone?!

Fast forward a few months. I got pregnant again. Hcg was fine. And sure enough, progesterone was in a healthy range. Sadly, the little bean implanted in my tube, rather than my uterus, so I had to interrupt the pregnancy, but it showed me that progesterone is not an issue for me.

I hope that you have resolution soon. I am sorry you are going through this.
