SA Results in, Morph ?

Just got a call from my RE herself, which has NEVER happened in the 6 months we have been there. Needless to say I almost fell off my chair when she asked if I had a minute to chat. She got the SA results back and didn't give me numbers but said the concentration and motility were good but the morph was low. She said they like to see 5% as normal and we are at 3%. What exactly does this mean? She said we will make a note of it considering we have needed ICSI in the past and leave it up to the embryologists the day of. Ive reached the point where we are doing IVF regardless so it doesn't matter if there are sperm issues. They can make the determination then. We just feel that we aren't qualified to make the ICSI determination, considering that the sample will be different in June then it was last week.

  • I think considering Morph is the least important, and your number is 3...and not say zero....that your RE's advice is great!  Let the embryologist decide day of.

    OR you could do what one of our girls did (I can't remember who it was) that let them do ICSI with a few eggs and not with a few others (depending on how many mature eggs they retrieve) and see how they do.

  • I was always told the same thing - based on our SAs we would need ICSI.  But it depending on the sample the day of they would decide whether to do all ICSI or split them.  We ended up doing ICSI for all of our IVFs.    Leaving it up to the embryologist is the best decision - they are the experts.

  • I would just ICSI all of them to be safe. ICSI has better fert rates than conventional anyway, so if there is any question, use ICSI. Worst case scenario is that you try conventional and 0% fertilize. Rescue ICSI afterword rarely works. To me, that's not a risk worth taking.

    You could leave it up to the embryologist too, but most likely he will ICSI all of them unless my some miracle, the morph suddenly jumps a few %. For us, our morph was always very consistent over several months.

  • Thanks for the advice girls. This really hasnt been a problem in the past and we never had fert problems before. Some needed ICSI and some didn't. We will probably need it but she said they are very conservative. I am sure given my past they will make the best decision. Im trying not to stress over it too much but am glad most of you agree!

    At my clinic, the cutoff for ICSI vs. conventional IVF is < or = to 4% morph.  We didn't even know DH had morph issues until our first IVF cycle, when they told us they did ICSI on all the eggs because his morph was only 3%.  They didn't give us an option at all - this was just the way the embryology lab operates, less than 4% = ICSI.  Personally, I didn't mind, because we had 100% fertilization that cycle.  All our other cycles were ICSI after that, and we had near 100% fertilization those cycles as well.  I guess I feel like the bottom line is to do whatever is going to give the best results - and if your RE thinks ICSI will give the best results, I'd do it.   :)

    04/07/11- PCOS Diagnosis w/GYN. Put on Metformin (1500 mgs)
    04/25/11- 1st consultation with RE (Confirmed PCOS & Anovulation)
    05/09/11-HSG-All clear! 05/13/11-SA-Normal. Found out on 2/14/12, low Morph (1%).
    06/17/11-10 days of Clomid (100, 150) No response.
    07/10/11-03/20/12- 5 Injectable (Follistim) IUI cycles; 4 BFN, 1 C/P. (stims ranged from 11 days to 25 days)
    05/28/12-Diagnosed with Hypothyroidism. 75 mcg Synthroid.

    IVF with ICSI in June/July 2013 = BFP!. Beta # 1 = 123. Beta # 2 = 252. Due March 25th. Baby boy arrived March 27, 2013!

    FET #1 - 10/4/14 = BFP!! Beta #1 = 179. Beta # 2 = 499. Due June 22nd.

It seems they needed ICSI on one or two but I dont even know if those were the ones we transferred. The second time we needed ICSI on all and had the exact same results! 18 retrieved and 10 fertilized! So at this point we are just seeing what happens. I think what you said is pretty much true and that on the day of if the morph is still at 3% they will probably just ICSI them. I told her i'd rather that so we can get them all to fertilize and have more to work with, i mean after all this! lol So we shall see. Thanks again hun, how are you feeling?

    I'm feeling OK.  Morning sickness is making a comeback since this weekend, but really, I can't complain!

It took me a long time to get comfortable with the way they do things and Im still gettting used to it. At this point we are like whatever gets us a baby! LOL Let me know what your calendar is like! I really like my Dr. I came from a much smaller practice with 3 Drs & really liked that, but they missed giving me some important info (like extremely low Morph) & they were more expensive than RMA to do the IVF. We did our 1st & only iu with them (5th total) to let them see for themselves how we stim (beta on Thur) & then onto IVF.

I also looked at their success rates and they are excellent so Im hoping to fall on that side of the statistics for once!  So when are we having a GTG??????

  • We dont get to make that decision at our clinic.  Its up the the embryologist.  My RE told me its something they do only if they have to and its something that can only be determined while in the lab.

    That being said, we only have slight morph issues so its probably not something they are expecting to do.  If we had different MFI issues, maybe we would get a say in the matter.

    IVF #1= BFP. m/c at 7w6d. Needed 2 D&C's and scar tissue removal. Mild OHSS
    IVF #2 = BFP. Severe OHSS. 4 Drainings. TWINS!
