I have the Ergo and have never used it for both kids. My babies are huge and just too heavy to carry since I don't have a great back anyway. I also can't hold my toddler if he starts to throw a tantrum or I need to pick him up for some reason. I also don't have a double stroller yet (my kids are 2 and 4 months). Just haven't needed it yet. Anywhere we've done a lot of walking, like a theme park or the mall, I have another member of my family there so 2 single strollers works fine.
I've learned with my second that there really isn't much that I can't do without. My second doesn't take bottles or pacis even. Every situation is different. For me, things I can't live without are the Rainforest Bouncer (only thing either of my kids would stay in while I get stuff done in the house), the Angelcare Monitor since both would only sleep on their tummies, the Arms Reach cosleeper so my babies can sleep in my room and because the Angelcare doesn't work if they move too much in their cribs, a big diaper bag, white noise, and my nursing cover. Pretty much everything else I can take or leave.

I wore the ergo comfortably with 2 slipped discs/SI joint issues. I still even wear my 33lb toddler and it's comfortable. Maybe you didn't adjust it properly? I didn't find it comfortable at first either until I really got used to it.
My two are 17 months apart. We have the double stroller, we have the carrier....but my most needed item was our P'n'P set up in the living room to put DD2 in while out of the room so that DD1 couldn't accidentally injure her. A 17 month old rambunctious toddler just doesn't understand to be gentle and cannot be around baby unsupervised at first. This way I could go to the bathroom, make dinner, etc.
It was a long road for our family...our 3 angels in heaven are always watching over us!!!
Re: #1 recommended item for 2under2

Ditto! Hands free is huge!

I don't hve 2 under 2 but I do have the Lilebaby... it is the Ergo. I love this.
Couldn't live without it with just one... great invention. Check them both out on Amazon.
