blister, what caused it?

My little man has a blister on the very tip of his penis, any idea what caused it? I change him often, and use inserts that are both natural and microfiber and fleece. If it is amonia caused or yeast caused how do I keep it from happening again. It doesn't seem to both him when I put medicine on it.
