IUI response

I know that this is a silly question- but I am confused.  What is considered a good response (in terms of numbers of follies) to IUI- for someone who is AMA.  Before I started stimming for IUI, my RE told me he did not want to see more than 2 follies from me (due to risk of multiples- I think?), but I am seeing posts of folks with 3-6 follies from IUI.  Should he be being more aggressive with the meds to produce more follies? Or is this only for IVF? This cycle, I was on femara then gonal-f (75 units from a 900 iu pen times 4 nights).  It seemed like a small gonal-f dose to me.

7/12: DE IVF # 1 (with ICSI)- 20R, 16M, 14F, 5DT of 2 blasts; 6 frosties = BFN;
Lupus anticoagulant initially high, then found to be normal on hematology consult;
Follow up testing in September all clear;
Started synthroid for "high normal" TSH;
FET # 1- late October 2012- BFP on FRER; beta # 1- 21(low), beta # 2- 48 (still low), beta # 3- 132, beta # 4- 1,293; beta # 5- 5,606; last beta- over 100,000. First u/s 11/21- heard heartbeat
12/12- Officially an OB patient!
Level 2 ultrasound at 20 weeks shows vasa previa and VCI
Referral to MFM and mandatory c section for delivery
Beautiful baby girl born at 34 weeks
Finally home after 15 day NICU stay!
Trying for sibling: FET # 2- May 2014; beta 5/31, BFN
FET #3, early July 2014; beta 7/14, BFN
DE IVF # 2- August 2014; 14R, 13M, 11F, 5dt of 2 blasts (3 AA), 5 frosties = BFN
FET #4- December 2014, yet another BFN

Dr. KK work up shows borderline uterine blood flow, elevated NK cells, and MTHFR mutation (homozygous for c677t)

Added baby aspirin, prednisone, supplements, Metanx, and intralipids

Switched to large clinic for final attempt; had endometrial receptivity testing in January; FET March 2015 = yet another BFN

  • I'm not sure how many follies is a good number, but I am doing my first IUI.  I'm taking Clomid 100mg (cycle day 4-9) and Follistim 50mg.(cycle day 6-11).  I have 4 follies right now.  My RE said he doesn't want to see a lot of follies because of multiples.  I go in for another monitoring appt on Thursday.  I had asked the nurse about the dose and she said for IUI they keep it lower.

  • Unless there is MFI present, usually 1-2 follies is the goal. For MFI it's 3, sometimes 4. Every RE has different cutoff points.

  • My RE originally said the max she would let me move forward with was 2-3.  After several failed attempts with 3 follies she let me go ahead with 4-5.  It made me really nervous but it didn't work.

    After more than 2 years of fertility treatments, FET did the trick!
    IVF March 2012 - BFP! - Severe OHSS = 8 days in the hospital in kidney failure

  • For my IUI, I had 3 mature ones (2 20s and one18) and 4 immature ones (16 and less) and that was all my RE wanted (18 is "mature" to him). I think you want to look at the number of mature follicles, not total. Also, these little buggers and come and go all day long so just because you may have 4 today doesn't mean they will all be there on your next scan.

    As far as IVF goes, my RE wants to see as many as we can get so hopefully I can have some left over and get a good number to work with. IVF is more controlled than IUI which is the reason for the differ enc in the numbers. It is all still a crap shoot though...

  • I think it also has to do with your age. Women under 35 have greater chance of multiples than say women over 40. I'm 42 and have had 2 IUI's. My RE was very aggressive with my med protocol and I had 4 follies the first and 5 the second, both failed =( .  But we realized it was due to MFI, so now we're doing IVF and have 5 strong growing embies.. going in for my ET today, so nervous!!  Anywhoo, so yeah to answer your question.. I think it depends on the individual, their age, if there is MFI present, or any other factors.  Godd luck Chicagobride07.. we'll all be rootin' for ya!
  • Thanks, everyone.  Cyberchop- best of luck with the IVF!  Do you mind sharing what meds you were on for your IUIs?  I am 42 as well.  =)
