We love InchBug labels - the Orbit labels and the sticky labels have worked perfectly for us.

Its a plus that they're locally based.

Mytwobabes on etsy.  It cost me about $6 (each) for a pack that will last each kid through elementary school.  The same labels that I stuck on DS's water bottles/lunch dishes for preschool 3 years ago are still going strong, that's with 3 trips a week through the dishwasher.  I haven't used them on items that I launder, but I did stick them in DD's tap and ballet shoes at the beginning of the school year and they have yet to budge, even on fabric/leather.
