Bf-ing while Teething/reflux help

DS is almost 5m and is starting to teeth he develeoped a little reflux this week from all of the saliva that he is producing and swallowing so feedings have been really hard he thrashes around and screams and I try my best to keep him sort of upright which is hard to do when you're bf-ing.  Also the teething is getting bad and both of my nipples look like he took a bite out of them, one is sort of bruised and the other looks all welted when he pulls off.  Any advice on how to get through this?
