Estradiol levels...how high is too high for IVF?

Sorry I've been MIA the past week or so. I've been feeling crazy awful with all the IVF meds and just feeling overall overwhelmed. UGH!

I went in for b/w and an u/s today. So far I've got about 25 or so follies that look like they are taking the "juice". Everything seems to look good so far and I have an estimated ER of either this Friday or Saturday...eek!!!

My question for those of you who have been through this....my estradiol level was 975 on Friday and it's 4091 today....how high it "too high" for them to cancel a cycle? I was supposed to take more Follistim tonight, but got an email from my nurse telling me to "coast" for the evening. I'm sure that if I was in trouble she'd tell me, but at the same time I can't help but feel a little nervous that I might overstimulate and get cancelled. Am I being stupid for getting worried?

  • Glad to hear you're having such a productive cycle!  You're not being stupid -- I believe (back when I was worried about overstimming) that many REs cancel around 5,000.  That was just from online research though; I didn't have that conversation with me RE because I peaked around 3,000.  With that number, I had 27 mature eggs (I was on antagonist).  Again, that's just my memory -- best to double check with others.

    Anyway, you're probably ok for now, but I think it's good she's letting you coast!  I've also read that in high producing ladies coasting can actually enhance egg quality.  Here's hoping you get lots of beautiful eggs at ER!

    IVF #1 January 2012, ER Jan 14th: 34R, 27M, 23F. Day 3: 18 embies still strong. Day 5: zero "good," one "fair," the rest "poor." Transferred 3. None made it to blast or to freeze. Jan 28: BFN.

    Lucky IVF #2: Transferred two beautiful day three embies on St. Patrick's Day. BFP on HPT 7dp3dt. Beta 1 (14dpER)=106; Beta 2 (16dpER)=140; Beta 3 (19dpER)=264! First u/s 4.17.

    Thanks girl! I started freaking out from doing online research because I kept finding things like "approaching or over 4000" to cancel. I guess it just depends on your RE. I'm for more b/w tomorrow and then another u/s on Wednesday so I guess we'll see! Thanks for the info. I feel better. :)

  • Mine was around 5,700 when my RE put me on a day of Lupron to bring the # down. However, the next day it was almost 6,100.  Since I was not on a high does of injects and there was no guarantee this wouldn't happen on a future cycle, what we had decided to do on the 5,700 day was to do the ER, wait for my #s to come down, and do an FET.  We had 37 mature eggs and 27 fertilized.  ER was 12/6 and FET #1 was 1/22.  Though we transferred two perfect Grade 1 8-cells, FET #1 did not work.  FET #2 is 2/22.  Praying hard this one will work!!
    March 2011: Off Nuva, cycle back to "normal" for me: No periods since 15 years old. June 2011:Provera&50 mg Clomid; Progesterone:0.7 July 2011:Provera&100mg Clomid; Progesterone:3.29 Met with RE:No Clomid response, begin injectables Sep 5 mg Letrozole and Ovidrel in the interim month. Cut out running (was a distance-runner), cycling, eliptical. Restricted to weight-training, walking, pilates. Brain MRI normal. Being physically over-stressed is the reason the body stopped producing prog. Late Sep 2011: Menopur, Ovidrel,& IUI (10.10.11):BFN-Great injectable response: 2 mature, 6 near mature, many smaller; Problem: 9 cysts! Dr: IUI too uncontrolled for number of viable eggs & age. On to IVF! IVF ER 12.6: 37 mature eggs, 27 fertilized, froze all to avoid overstimulation; FET 1.22 (2 Grade 1)=BFN; FET 2.22 (3 Grade 2)=BFP!   • I went to retrieval between 6K-7K.  They cautioned me about OHSS and had me do a reduced trigger.  It really depends on what your RE thinks.  I would say if they haven't given you the cancellation/OHSS talk they're planning on letting you to retrieval.  Good luck!  I chugged gatorade and protein after ER and only had minor OHSS symptoms.  Mdluv (mostly on IFV now) got the opposite advice to limit fluids and avoided OHSS too.  I'm not sure which one of us got the right advice but we both lucked out.

  • I heard between 5000-7500 depending on the RE.  There are options to prevent an all out cancel such as coasting, Lupron trigger, or freezing and doing an FET.  Even if it does get too high they'll probably retrieve and monitor you with ultrasound leading up to the ET and see if you have free fluid before canceling since most cases of OHSS are developed between ER and ET.  Good luck!

  • I was around 4000 at time of trigger. We ended up with 14 mature eggs, and I felt like crap for about a week afterwards. No serious OHSS though. If they are really worried, they may do ER but then have you wait and do an FET later, because if you are at risk for OHSS and do get pregnant, the rising hCG will make it worse. But it sounds like you are still in a safe range...

  • Because my levels were high, my nurse told me to coast for the last day.  She also reduced my trigger shot (5000) and I still had my ER yesterday, which produced 17 eggs.  As a result of the high E2 levels, I won't be on estrace or vivelle following the ET.   It sounds like they just want to keep you in a safe place.   Good luck.
