Follicle size & triggering question

Obviously this would be much more clear if I was seeing an RE and having multiple u/s, but we're waiting 2 months and doing Clomid + TI with an OB to try and save money and hoping it miraculously works.

My first question is, what is a good follice size? I am going in on CD 12 to have my (one and only) u/s of the cycle and want to be sure I know what to listen/look for.

Second question - let's say 20 is good (just throwing that out there because I don't know), and I go in on CD 12 and I'm at a 15. Will they just estimate and tell me to trigger in another day or two? If they have me trigger that day with a 15, will that make me release an immature egg, thus having little chance of conception? If I'm right at 20, will I trigger right then? (I am assuming this is one of the exact problems with not having monitoring 2-3x like they do with an RE!!!)

  • firstly, you really should be going to a RE for this, butttttt this is not happening this cycle.  Clomid doesn't always work completely on schedule, so you might not be ready on cd 12.  Are they doing bloodwork too?

    your follicles normally grow 2 mm a day, so if you are 15, you will be ready to trigger in 2 days approximately. Also, a trigger makes you ovulate 36 hours later, fyi.

  • I really wish you were seeing an RE. I just did my first cycle of clomid 50mg + Trigger+ TI. At my  CD11 U/S My lining was still super thin and my follies were still pretty small. I went back in on CD13 for another U/S and had a beautiful follie, but still really thin lining. He gave me estrogen to take over the next two days and and had me trigger on CD15, hoping my lining would be thick enough.

    I don't know if this will help you, but this was my experience. I hope that you consider seeing an RE soon.

  • 20ish is about right, but like PP said, your lining might be thinned out from the clomid too. bloodwork will also say if you are ready to ovulate or not, they should be doing this when you get your u/s.

  • I went in in cd11, my follies weren't big enough yet (14 or less). They had me return 2 days later to make sure they were growing appropriately. My only good follie measured 17 so they told me to trigger 2 days later. I tested an opk that morning just to see and I was close to O'ing which put the trigger right on target. They weren't going to guess when to trigger after that first ultrasound incase I didnt continue to develop as I should have.

    I left that out. I did do Bloodwork on CD 5. DH did an SA and I had an HSG xray done. Clomid will not help if you have blocked tubes. My follie was 22cm.

  • Realy 36 hours later, then I am wondering why we are doing our IUI 12 hours after our trigger then the second one is 36 hours after the trigger

    I thought they grew 1 mm a day. If 2 mm is right, I have less bloodwork in my future than I was anticipating - YEAH!

  • I had a Friday apptmt where I had 1 follicle at 15.  When I came back on Monday I had 4 big ones at 20, 18, 15 and 13.  I think the tech told me 18 was what they looked for, so I triggered that night.

    Re: your other question about immature eggs, I have been trying to figure this out myself.  I don't understand what would have happened to my other 2 eggs that might have been immature.  Maybe I need to read up on anatomy or something...I find it difficult to get clear information about what's actually happening physiologically.

    You ovulate about 36 hours after. You have better chances if you have sex or IUI before ovulation. So DH and I are doing TI So I was told to have sex the night before trigger and 36 hours after trigger, and every other day.

  • At my office, a follicle measuring 17.6 or above is considered mature.  And from what I've been told, they continue to grow 1-2 mm per day, so they can't really adequately guess you'll be ready in 2 or 3 days.  If you are JUST under their cutoff, they may tell you to trigger the next day without coming in again for monitoring.  GL!!

  • I did a medicated IUI this cycle and had 2 great follicles on my right ovary measuring both around 15 last Thursday, CD10. My RE told me to take ovulation tests just to make sure. Well that night I got a smiley face, my RE could not believe it. I went in the next morning for a U/S and they had grown to 17 and 17.5 and my lining was 8.9. So we went ahead with the IUI at 12:30 pm, i was nervous that my follicles were not large enough but she said she felt that they looked good.. My point is it is very hard to know how the body is going to react to meds and so on. I ovulate regularly and usually every 12-14 days, this was VERY early for me. This is how being monitored help, if i was not being monitored I would of missed my ovulation. I wish you good luck.
